Abnormal genital bleeding?
Hello Doctor, I would like to ask you about my situation.
My last menstrual period started on December 29 and ended on January 6 (my periods usually last 8-9 days, but my cycle has always been irregular, ranging from 28 to 40 days).
This week, my period was expected to start on Monday or Tuesday, but it hasn't arrived yet (the last time I had intercourse was on February 8), and today marks 45 days since my last period.
On the evening of February 10, I noticed three small spots of light red bleeding (not bright red) on my underwear, distributed in different areas.
The next day, February 11, I had a small amount of bleeding again, and today, February 12, I experienced a small amount of bleeding, but around noon, I suddenly had a drop of heavier bleeding, followed by more small amounts.
Since February 10, I have also been feeling a bit nauseous, unable to keep food down, and have no appetite.
I read online that if bleeding occurs outside of menstruation, it could be due to hormonal imbalance, cervical cancer, cervical polyps, etc.
However, if pregnancy is confirmed, there can be light bleeding due to implantation.
I haven't taken a pregnancy test, so I'm unsure if I am pregnant.
I would like to ask the doctor why I am experiencing this bleeding, as it seems unusual; I have never had bleeding before my period in the past.

Hello, the information you provided does not allow for a definitive determination of whether you are experiencing menstruation.
Early pregnancy may not present specific symptoms, and subjective symptoms alone cannot be used for diagnosis.
There are many causes of bleeding outside of menstruation (such as lesions in the reproductive organs, pregnancy, hormonal imbalances, etc.).
I recommend that you consult a gynecologist to arrange for urine tests, ultrasound, or blood tests to determine the cause of the irregular bleeding.
Thank you! Wishing you peace and health.

More Info
Abnormal vaginal bleeding can be a source of significant concern for many women, as it may indicate underlying health issues or hormonal imbalances. Based on your description, it appears you are experiencing irregular bleeding that is not consistent with your typical menstrual cycle. Let's break down the potential causes and considerations regarding your symptoms.
Possible Causes of Abnormal Vaginal Bleeding
1. Hormonal Imbalance: One of the most common reasons for abnormal bleeding is hormonal fluctuations. The menstrual cycle is regulated by a complex interplay of hormones, including estrogen and progesterone. If these hormones are out of balance, it can lead to unexpected bleeding, such as spotting between periods or changes in the menstrual cycle's regularity.
2. Ovulation Bleeding: Some women experience light spotting during ovulation, which typically occurs about midway through the menstrual cycle. This is often referred to as ovulation bleeding and is usually light and short-lived. Given that your cycle can vary significantly, this could be a possibility.
3. Pregnancy-Related Bleeding: If there is a chance of pregnancy, it is important to consider implantation bleeding, which can occur when a fertilized egg attaches to the uterine lining. This type of bleeding is usually light and can be mistaken for a light period. Given that you mentioned a recent sexual encounter, it would be prudent to take a pregnancy test to rule out this possibility.
4. Cervical or Uterine Issues: Conditions such as cervical polyps, fibroids, or even infections can lead to abnormal bleeding. These conditions can cause changes in the uterine lining or cervical tissue, leading to unexpected bleeding. If you have not had a recent pelvic exam, it may be beneficial to consult a healthcare provider for further evaluation.
5. Stress and Lifestyle Factors: Stress, significant weight changes, and lifestyle factors can also impact your menstrual cycle and lead to irregular bleeding. If you have experienced any recent changes in your life, such as increased stress or changes in your routine, these could contribute to your symptoms.
Recommendations
1. Pregnancy Test: Given the possibility of pregnancy, it is advisable to take a home pregnancy test. If the test is positive, you should schedule an appointment with your healthcare provider for further evaluation and care.
2. Consult a Healthcare Provider: Regardless of the pregnancy test result, if the abnormal bleeding persists or worsens, it is important to consult a healthcare provider. They may recommend a pelvic exam, ultrasound, or other diagnostic tests to determine the cause of the bleeding.
3. Monitor Symptoms: Keep track of your bleeding patterns, including the amount, color, and any associated symptoms such as pain or nausea. This information can be helpful for your healthcare provider in diagnosing the issue.
4. Lifestyle Considerations: Evaluate your lifestyle for any factors that may contribute to hormonal imbalances, such as diet, exercise, and stress management. Maintaining a healthy lifestyle can support overall hormonal health.
Conclusion
Abnormal vaginal bleeding can stem from various causes, ranging from hormonal imbalances to potential pregnancy. It is essential to take your symptoms seriously and seek medical advice, especially if the bleeding continues or is accompanied by other concerning symptoms. By consulting with a healthcare provider, you can receive a proper diagnosis and appropriate treatment to address your concerns. Remember, your health and well-being are paramount, and timely intervention can lead to better outcomes.
