Questions related to high myopia?
Hello Doctor: I previously inquired about issue #178383.
I have high myopia (before dilation L975, R1250) and also experience floaters.
During my most recent examination, the doctor mentioned that the fundus examination appeared normal, but the OCT report indicated that the retina and macula in my right eye are thinner.
I would like to ask the doctor: (1) Does the retina and macula continue to thin with age? Is it possible to maintain their thickness? (2) Last time you mentioned that retinal degeneration and macular degeneration can lead to certain conditions.
If such unfortunate events occur, I would like to know if there are currently any treatment options available.
Thank you for your response, Doctor!

Dr. Gao Pilin reply Ophthalmology
Hello JJ: 1.
It is possible to maintain the current condition; however, with high myopia, the retina may be less healthy, and there is a possibility of degeneration occurring more rapidly with age compared to the general population.
2.
Degeneration of the retina and macula can severely affect vision.

High myopia, or severe nearsightedness, is a condition that significantly increases the risk of various ocular complications, particularly involving the retina and macula. As a patient with high myopia, it is essential to understand the implications of your condition and the potential changes that may occur over time.
1. Does the retina and macula continue to thin with age? Can it stabilize?
Yes, both the retina and the macula can thin as part of the natural aging process, especially in individuals with high myopia. This thinning is often due to structural changes in the eye, including elongation of the eyeball, which can lead to increased tension on the retinal layers. While some patients may experience stabilization of these changes, others may continue to see progressive thinning. Regular monitoring through comprehensive eye exams, including OCT (Optical Coherence Tomography), is crucial to assess the health of the retina and macula over time. Maintaining a healthy lifestyle, managing systemic health conditions (like diabetes and hypertension), and avoiding eye strain can contribute to better overall eye health, but there is no guaranteed way to prevent thinning.
2. What are the implications of retinal and macular degeneration, and are there treatment options?
Retinal and macular degeneration can lead to various symptoms, including blurred vision, distortion, and in severe cases, vision loss. In high myopia, the risk of conditions such as myopic macular degeneration, retinal tears, and retinal detachment increases. If degeneration occurs, treatment options depend on the specific condition and its severity. For instance:
- Myopic Macular Degeneration: This may be managed with anti-VEGF (vascular endothelial growth factor) injections, which can help reduce fluid accumulation and preserve vision.
- Retinal Tears or Detachment: These conditions often require surgical intervention, such as laser photocoagulation or vitrectomy, to repair the retina and prevent further vision loss.
- Monitoring and Lifestyle Adjustments: Regular follow-ups with your eye care provider are essential. Lifestyle changes, such as protecting your eyes from UV light, maintaining a healthy diet rich in antioxidants, and managing systemic health issues, can also play a role in preserving vision.
In conclusion, as a high myopia patient, it is vital to stay vigilant about your eye health. Regular eye examinations, including OCT and dilated fundus exams, are crucial for early detection of any changes in the retina or macula. While some degree of thinning may be inevitable with age, proactive management and timely treatment of any emerging issues can significantly impact your visual prognosis. Always consult with your eye care professional regarding any changes in your vision or concerns about your eye health, as they can provide personalized advice and treatment options tailored to your specific situation.
