Menstruation has not occurred?

Dear Doctor, I had sexual intercourse on December 25th with a condom, and I checked afterward to confirm that it did not break.
On January 5th, during my ovulation period, there was some foreplay but no penetration, and I am unsure if any semen came into contact with my vulva.
My period came from January 19th to 24th, but I have not had my period in February, even though my menstrual cycle is usually regular (26 days).
Could this be due to pregnancy causing my missed period? I would appreciate your help in clarifying this.
Thank you.

Emily, 20~29 year old female. Ask Date: 2022/02/16

1.
For pregnancy concerns, please take a home pregnancy test or visit a gynecologist for examination and evaluation four weeks after your most recent sexual intercourse.

2.
The menstrual cycle can be affected by various psychological and physiological factors.
If your period is more than two months late, it is recommended to see a gynecologist for examination and evaluation.

It sounds like you're concerned about a missed period and whether it could be due to pregnancy after having safe sex. Let's break down your situation and provide some clarity.

First, you mentioned that you had sexual intercourse on December 25th, using a condom throughout the encounter. You also checked the condom afterward to ensure it did not break, which is a crucial step in ensuring safe sex. The fact that you used a condom correctly significantly reduces the risk of pregnancy. Condoms, when used properly, have a failure rate of about 2% to 5%, meaning that out of 100 couples using condoms as their only form of birth control for a year, 2 to 5 will experience an unintended pregnancy.

You also noted that your ovulation period was around January 5th, and while you engaged in some intimate activities, there was no penetration, and you were unsure if any semen came into contact with your vulva. This is an important detail because the risk of pregnancy is significantly lower when there is no ejaculation near the vaginal opening. Pre-ejaculate fluid can contain sperm, but the chances of pregnancy from this fluid are much lower compared to direct ejaculation.

Your menstrual cycle has been regular, typically around 26 days, and you mentioned that your period was due around January 19th to 24th. Since you have not had your period in February, it is understandable that you are concerned about the possibility of pregnancy. However, there are several factors that could contribute to a missed period aside from pregnancy:
1. Stress: Emotional or physical stress can affect your hormonal balance and lead to a delayed or missed period.

2. Changes in Weight: Significant weight loss or gain can also disrupt your menstrual cycle.

3. Illness: Any recent illness or medical condition can impact your cycle.

4. Hormonal Imbalances: Conditions like polycystic ovary syndrome (PCOS) or thyroid issues can lead to irregular periods.

5. Birth Control: If you have been using any hormonal birth control methods, they can also affect your cycle.

Given that you have had safe sex practices, the likelihood of pregnancy is low, but it is not impossible. If you are still concerned, the best course of action would be to take a home pregnancy test. These tests are quite accurate when taken after a missed period. If the test is negative and your period still does not arrive, or if you have any other concerning symptoms, it would be advisable to consult with a healthcare provider for further evaluation.

In summary, while the risk of pregnancy appears low based on the information you've provided, several factors could explain your missed period. Taking a pregnancy test will provide clarity, and if necessary, seeking medical advice can help address any underlying issues that may be affecting your menstrual cycle. Remember, maintaining open communication with your healthcare provider about your reproductive health is essential for managing your concerns effectively.
