The difference between cauliflower and sebaceous gland hyperplasia?
Hello, a while ago during intercourse with my girlfriend, she noticed that I had several raised spots on the inner skin of my foreskin.
After a week, I visited a dermatologist, who examined me and said it was sebaceous gland hyperplasia.
I requested to have tests done, but he said it wasn't necessary and prescribed me a topical medication called Fucicort.
It's been almost a year since my last sexual partner, and I had similar growths several years ago, but they haven't increased in size or number.
However, this time, I can feel them more noticeably during an erection.
Should I be concerned? What is the difference between genital warts and sebaceous gland hyperplasia? Thank you.

Pearly penile papules (PPP) and sebaceous gland hyperplasia (SGH) are two distinct conditions that can occur on the penis, and understanding the differences between them is important for both diagnosis and management.
Pearly Penile Papules (PPP):
PPP is a common and benign condition characterized by small, dome-shaped, flesh-colored or white papules that typically appear around the corona of the glans penis. They are usually arranged in one or two rows and can vary in number from a few to several dozen. PPP is considered a normal anatomical variant and is not associated with any disease or infection. They are often mistaken for sexually transmitted infections (STIs) due to their appearance, but they are completely harmless and do not require treatment unless the individual desires cosmetic removal for personal reasons.
Sebaceous Gland Hyperplasia (SGH):
SGH, on the other hand, refers to the enlargement of sebaceous glands, which are responsible for producing sebum, an oily substance that helps lubricate the skin. In the context of the penis, SGH can present as small, yellowish or skin-colored bumps that may appear on the shaft or glans. These lesions are also benign and are often mistaken for other conditions, including STIs. SGH can occur due to various factors, including hormonal changes, skin irritation, or genetic predisposition. Like PPP, SGH does not require treatment unless it causes discomfort or the individual wishes to have them removed for aesthetic reasons.
Key Differences:
1. Location: PPP typically appears around the corona of the glans penis, while SGH can occur on various parts of the penis.
2. Appearance: PPP is characterized by small, dome-shaped papules, while SGH appears as yellowish or skin-colored bumps.
3. Etiology: PPP is a normal anatomical variant, whereas SGH is related to the enlargement of sebaceous glands.
4. Symptoms: Both conditions are asymptomatic, meaning they do not cause pain or discomfort. However, if SGH becomes inflamed or irritated, it may lead to discomfort.
Management and Considerations:
If you have been diagnosed with SGH by a dermatologist and have been advised that no further testing is necessary, it is likely that the condition is benign. However, if you notice any changes in the size, shape, or number of lesions, or if you experience any discomfort, it is advisable to follow up with your healthcare provider.
Regarding your concern about the previous sexual partner and the recurrence of similar lesions, it is important to note that PPP and SGH are not sexually transmitted. However, if you have any doubts or concerns about sexually transmitted infections, it is prudent to get tested, especially if you have had multiple partners.
In summary, both pearly penile papules and sebaceous gland hyperplasia are benign conditions that do not pose a health risk. Understanding the differences can help alleviate anxiety and guide appropriate management. If you have further questions or concerns, consulting a healthcare professional for personalized advice is always a good approach.
