Gum problems

There is a white area on the outer side of my gums that hurts when toothpaste touches it.
Occasionally, there is slight bleeding, and it also hurts when I don't eat or drink and accidentally touch the sore.
The symptoms have been present for about a week.
I visited the dentist, but the doctor said it was due to brushing my gums and that it would heal in two to three days.
However, the sore has lasted more than three days, and now I don't know what to do.

Hello, the recurrent canker sores you are experiencing may be due to generally low immunity.
It is recommended to maintain a positive mood, follow a regular routine, and avoid excessive irritation to the affected area.
They should heal on their own in about 1 to 2 weeks.
Please refrain from smoking, and if you are taking hormones or immunosuppressive medications, be sure to inform your prescribing physician to consider stopping the medication as needed.
Sincerely, Lin Chia-Yung, Department of Dentistry, Taichung Hospital, Ministry of Health and Welfare.

Gum issues, such as pain, bleeding, and discomfort, can be quite distressing and may indicate underlying dental or periodontal problems. Based on your description, it seems that you are experiencing pain and occasional bleeding in the gums, particularly on the outer side, which has persisted for over a week. You mentioned that brushing your teeth, especially with toothpaste, exacerbates the pain, and that the dentist suggested it was due to brushing too hard. However, since the symptoms have not improved as expected, it is important to consider a few possibilities and treatment options.

First, the presence of white patches on the gums could indicate several conditions, including oral thrush (a fungal infection), leukoplakia (a condition that can lead to cancer), or simply irritation from brushing. The pain and bleeding you are experiencing could be due to gingivitis, which is inflammation of the gums often caused by plaque buildup. Gingivitis is characterized by swollen, red gums that may bleed during brushing or flossing. If left untreated, it can progress to periodontitis, a more severe form of gum disease that can lead to tooth loss.

Here are some steps you can take to address your gum issues:
1. Gentle Oral Hygiene: While it is important to maintain good oral hygiene, you should be gentle when brushing your teeth. Use a soft-bristled toothbrush and avoid aggressive brushing techniques. Consider using a toothpaste designed for sensitive gums, which may help reduce discomfort.

2. Rinse with Salt Water: A warm saltwater rinse can help soothe irritated gums and promote healing. Mix about half a teaspoon of salt in a glass of warm water and rinse your mouth several times a day.

3. Avoid Irritants: If you smoke or use tobacco products, consider quitting, as these can exacerbate gum issues. Additionally, avoid spicy or acidic foods that may irritate your gums further.

4. Over-the-Counter Pain Relief: If the pain is significant, over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate discomfort. However, be sure to follow the recommended dosage instructions.

5. Consult a Specialist: Since your symptoms have persisted beyond the initial assessment by your dentist, it may be beneficial to seek a second opinion from a periodontist, a specialist in gum diseases. They can provide a more thorough examination and may recommend treatments such as scaling and root planing, which involves cleaning below the gum line to remove plaque and tartar buildup.

6. Monitor Symptoms: Keep an eye on your symptoms. If you notice any changes, such as increased swelling, pus, or fever, seek medical attention promptly, as these could indicate a more serious infection.

In summary, while your dentist's initial assessment may have been correct regarding brushing technique, the persistence of your symptoms suggests that further evaluation is warranted. Maintaining gentle oral hygiene practices, using home remedies like saltwater rinses, and consulting with a dental specialist can help address your gum issues effectively. Remember, early intervention is key to preventing more serious dental problems in the future.
