Emergency contraception and menstrual sexual activity?
Hello doctor, I had unprotected sexual intercourse with my boyfriend on February 10th, but there was no ejaculation, and it only lasted for a short time (my period was from February 4th to February 9th).
However, I took the emergency contraceptive pill the next day.
I started bleeding on February 17th and have had some bleeding until today, February 21st.
Is this bleeding my period or a side effect of the pill? Additionally, I had unprotected sexual intercourse with my boyfriend again on February 20th while still bleeding, but there was no ejaculation.
Is there a chance of pregnancy in this situation? Thank you.
Ka, 20~29 year old female. Ask Date: 2022/02/22
Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the questioner: There is a possibility of pregnancy even if ejaculation has not occurred during intercourse, as pre-ejaculatory fluid may contain sperm.
The effects of emergency contraception on menstruation can vary; it may cause the period to come earlier or later.
Additionally, the effectiveness of emergency contraception is not 100%, with a failure rate of about 3%.
It is recommended to check for pregnancy if menstruation is more than a week late.
Thank you! Wishing you peace and health.

When it comes to understanding the relationship between post-pill bleeding and the risks of pregnancy after unprotected sex, it's essential to consider several factors, including the timing of your menstrual cycle, the use of emergency contraception, and the nature of the sexual activity.
Firstly, you mentioned that your last menstrual period was from February 4 to February 9, and you had unprotected sex on February 10. Since you took the emergency contraceptive pill (ECP) the following day, it is crucial to understand how these pills work. ECPs are most effective when taken as soon as possible after unprotected intercourse, ideally within 72 hours. They primarily work by preventing ovulation; if ovulation has already occurred, they may not be effective.
The bleeding you experienced starting on February 17 could be a side effect of the ECP. Many women experience what is known as "withdrawal bleeding" after taking emergency contraception, which can resemble a menstrual period but is not necessarily the same. This bleeding can occur anywhere from a few days to a few weeks after taking the pill and may last for several days. It is not uncommon for this bleeding to happen even if you are not pregnant, as it is a response to the hormonal changes induced by the ECP.
Regarding your question about the risk of pregnancy after having unprotected sex on February 20 while still experiencing bleeding, the likelihood of becoming pregnant is relatively low. Since you took the ECP after the initial unprotected encounter, and given that you are still experiencing bleeding, it suggests that the pill may have worked to prevent ovulation. Additionally, if there was no ejaculation during the sexual encounter on February 20, the risk of pregnancy decreases further.
However, it is important to note that while the chances of pregnancy are low, they are not zero. If you have concerns about potential pregnancy, it is advisable to take a home pregnancy test about two weeks after the last unprotected sexual encounter or consult with a healthcare provider for further evaluation.
In summary, the bleeding you are experiencing is likely a result of the emergency contraceptive pill rather than a sign of menstruation. Engaging in unprotected sex while experiencing this bleeding does carry a lower risk of pregnancy, especially if there was no ejaculation. However, for peace of mind, consider taking a pregnancy test if your next expected period is delayed or if you have any unusual symptoms. Always consult with a healthcare professional for personalized advice and guidance regarding your reproductive health.
