Gastrointestinal issues
Hello, about a year or two ago, I started experiencing abdominal pain before having a bowel movement, which often leads to belching.
My stool is generally loose and may contain food remnants.
In recent months, due to changes at work, I have been under significant stress, and my symptoms have worsened.
I occasionally feel discomfort in my abdomen, and my stool has been yellow, resembling watery or egg drop consistency, occurring about two to three times a day.
A month ago, I had an episode of gastroenteritis, during which I passed sticky, dark green stools approximately five times.
Since that day, my gastrointestinal discomfort seems to have increased; I often experience bloating after eating, and sometimes I have cramping abdominal pain before using the restroom.
Occasionally, I feel sharp pain on the left or right side of my abdomen for a few seconds, along with gas and bowel sounds, which usually do not have a strong odor.
Sometimes, after swallowing air, I can hear bowel sounds, especially when lying flat at night.
Upon waking, I feel some abdominal pain but tend to wait until I'm at work to have a bowel movement.
After eating, I often feel the urge to use the restroom.
After taking probiotics, my stool has started to form more consistently, but sometimes it appears as 2 cm long formed pieces, other times as a lump, or egg drop-like, still containing food remnants.
Occasionally, if I have a slight urge to go, I will sit down and produce a small formed piece.
If I eat leafy greens and pig liver the day before, I notice a bit of black residue when wiping.
Generally, I have bowel movements about twice a day, once after breakfast and once after lunch.
Recently, I have felt anxious about this situation, constantly thinking about it, and I have also experienced pain in various parts of my body.
I would like to ask what might be going on.
Thank you!

Hello, you may need to examine your dietary habits and mental stress to see if there are areas for improvement.
We cannot rule out the possibility of irritable bowel syndrome.
If there is no improvement, you should consult a gastroenterologist to rule out any underlying organ issues.

More Info
Understanding gastrointestinal issues can be quite complex, especially when multiple symptoms are present. Based on the information provided, it appears that you are experiencing a range of gastrointestinal symptoms, including abdominal pain, changes in stool consistency, increased gas, and a sense of urgency to defecate after eating. These symptoms can be indicative of several underlying conditions, including functional gastrointestinal disorders such as Irritable Bowel Syndrome (IBS), food intolerances, or even stress-related gastrointestinal disturbances.
Symptoms Breakdown
1. Abdominal Pain and Discomfort: The abdominal pain you experience, particularly before bowel movements, could be related to increased intestinal motility or spasms, which are common in IBS. The pain that subsides after a bowel movement is also characteristic of this condition.
2. Changes in Stool Consistency: The variability in your stool—from loose, watery stools to more formed shapes—suggests a fluctuation between diarrhea and constipation, which is typical in IBS. The presence of undigested food particles, like "菜渣" (vegetable residue), may indicate that your digestive system is not fully breaking down certain foods, possibly due to rapid transit time or food intolerances.
3. Increased Gas and Bloating: Symptoms such as belching and a feeling of bloating after meals can be attributed to the fermentation of undigested food in the intestines, which is common in individuals with IBS or food intolerances.
4. Stress and Anxiety: You mentioned increased stress at work, which can significantly impact gastrointestinal function. The gut-brain connection is well-documented, and stress can exacerbate symptoms of IBS or lead to gastrointestinal discomfort.
5. Recent Gastroenteritis: The episode of gastroenteritis you experienced could have altered your gut flora or led to lingering symptoms, such as increased sensitivity or altered bowel habits.
Recommendations
1. Dietary Modifications: Consider keeping a food diary to identify any potential food triggers. Common culprits include dairy, gluten, high-fat foods, and certain vegetables that can cause gas (like beans and cruciferous vegetables). A low FODMAP diet, which restricts fermentable carbohydrates, may also be beneficial for managing IBS symptoms.
2. Probiotics: Since you mentioned that taking probiotics helped with stool formation, continuing with a high-quality probiotic may support gut health and balance your intestinal flora.
3. Stress Management: Incorporating stress-reduction techniques such as mindfulness, yoga, or regular exercise can help alleviate gastrointestinal symptoms. Cognitive-behavioral therapy (CBT) has also been shown to be effective for managing IBS symptoms related to stress.
4. Regular Eating Schedule: Try to maintain a regular eating schedule and avoid large meals, which can exacerbate symptoms. Eating smaller, more frequent meals may help.
5. Consult a Gastroenterologist: Given the complexity of your symptoms, it may be beneficial to consult a gastroenterologist for a comprehensive evaluation. They may recommend further testing, such as stool studies, breath tests for lactose intolerance or small intestinal bacterial overgrowth (SIBO), or even imaging studies if warranted.
Conclusion
Your symptoms suggest a functional gastrointestinal disorder, likely exacerbated by stress and dietary factors. While it can be concerning to experience these symptoms, many individuals with similar presentations find relief through dietary changes, stress management, and sometimes medication. It’s essential to work closely with a healthcare provider to tailor a management plan that suits your specific needs and to rule out any serious underlying conditions. Remember, you are not alone in this, and with the right approach, many people successfully manage their gastrointestinal issues.
