Is Your Heartbeat Irregular? Palpitations and Their Causes - Cardiology

Is this arrhythmia?

Hello Doctor, I have recently been working out at the gym and using a smartwatch to monitor my heart rate, which is around 100 bpm, peaking at 148 bpm.
However, during exertion, I experience a sensation of skipped beats and a bit of palpitations, although I don't feel uncomfortable.
I've actually had this sensation of skipped beats for a while, but the frequency is low.
Last summer, when I was running, my heart rate was fast, and I also experienced skipped beats easily.
Since receiving the AZ vaccine, I've often felt palpitations, especially during my afternoon naps when I feel my heart racing and can't fall asleep, and sometimes this happens at night as well.
I would like to ask the doctor if there could be any heart issues, and whether I need to seek medical attention.

Many factors in daily life can contribute to this condition, such as emotions, stress, irregular sleep patterns, or excessive intake of caffeinated beverages.
It is recommended to adjust your lifestyle and engage in regular exercise.
However, if symptoms persist, it is advisable to visit a hospital for an electrocardiogram (ECG) to determine whether there is an arrhythmia.

Experiencing irregular heartbeats, commonly referred to as palpitations, can be concerning, especially when accompanied by feelings of anxiety or discomfort. Based on your description, it seems you have been monitoring your heart rate during workouts and have noticed episodes of what feels like skipped beats or a racing heart. This can be a common experience, particularly in individuals who engage in high-intensity exercise or have recently undergone vaccination, such as the AZ vaccine you mentioned.

Palpitations can be caused by a variety of factors, including physical exertion, stress, anxiety, caffeine intake, and even certain medications. In your case, the combination of weight training and the potential effects of the vaccine could be contributing to your symptoms. It is not uncommon for individuals to experience heightened awareness of their heart rate after vaccination, as the immune response can sometimes lead to temporary changes in heart rhythm.

When you mention feeling a "leak" in your heartbeat, it is important to differentiate between true arrhythmias (irregular heartbeats) and the sensation of palpitations that can occur due to anxiety or increased physical activity. In many cases, palpitations are benign and do not indicate a serious underlying heart condition, especially in younger individuals without a significant history of heart disease.

However, there are certain signs and symptoms that warrant further evaluation by a healthcare professional. If you experience any of the following, it is advisable to seek medical attention:
1. Persistent or worsening symptoms: If the palpitations become more frequent or severe, or if you experience them at rest rather than only during exercise.

2. Associated symptoms: If you experience chest pain, shortness of breath, dizziness, or fainting spells alongside the palpitations.

3. Family history: If there is a family history of heart disease or arrhythmias, it may be prudent to get checked.

4. Duration: If the palpitations last for an extended period or are accompanied by other concerning symptoms.

In terms of management, if your palpitations are determined to be benign, lifestyle modifications can be beneficial. These may include reducing caffeine and alcohol intake, managing stress through relaxation techniques, ensuring adequate hydration, and maintaining a regular exercise routine that is appropriate for your fitness level.
If you continue to experience palpitations or if they are causing you significant anxiety, it may be helpful to consult with a cardiologist. They may recommend tests such as an electrocardiogram (ECG), Holter monitor, or echocardiogram to assess your heart's rhythm and function more closely.
In summary, while your symptoms may be related to normal physiological responses to exercise or anxiety, it is always best to err on the side of caution. Monitoring your symptoms and seeking medical advice when necessary can help ensure that any potential heart issues are addressed promptly.
