Testicular discomfort or pain?

Hello, doctor.
I have been experiencing a dull sensation in my left testicle for two consecutive days, and the next day the right testicle also felt dull, with the sensation extending to my lower right abdomen.
However, the left testicle has no symptoms.
I have not been ill or had a fever recently.
Could this be varicocele? Additionally, is it normal to see veins when pulling the scrotal folds flat? Thank you for your response.

Mr. Lin, 10~19 year old female. Ask Date: 2022/03/07

Dr. Du Shixi reply Urology

Hello: It is not necessarily varicose veins.
If there is swelling, it is important to seek medical attention.
Avoid prolonged sitting or standing, and refrain from intense exercise for the time being.
By the way, is it normal to see veins when the folds of the scrotum are stretched flat? --> Yes, it is normal.
Wishing you good health.

More Info

Testicular discomfort can be concerning and may arise from various underlying conditions. Based on your description, it seems you are experiencing a dull sensation in both testicles, with the right side also radiating discomfort to the lower abdomen. While it is possible that this could be related to conditions such as varicocele (enlargement of the veins within the scrotum), it is essential to consider other potential causes as well.

Possible Causes of Testicular Discomfort:
1. Varicocele: This is a common condition where the veins within the scrotum become enlarged, similar to varicose veins in the legs. It can cause a dull ache or discomfort, particularly after prolonged standing or physical activity. Varicoceles are often more prominent on the left side due to anatomical differences in venous drainage.

2. Epididymitis: This is an inflammation of the epididymis, often caused by infection. Symptoms may include pain, swelling, and tenderness in the affected testicle, along with possible fever or discharge.

3. Testicular Torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off the blood supply to the testicle. Symptoms typically include sudden, severe pain and swelling. If you experience acute pain, it is crucial to seek immediate medical attention.

4. Inguinal Hernia: This occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. It can cause discomfort in the groin and lower abdomen, and may be more noticeable when standing or straining.

5. Trauma or Injury: Any recent injury to the groin or testicles can lead to discomfort and should be evaluated.

6. Testicular Cancer: Although less common, a lump or mass in the testicle can cause discomfort and should be investigated.

Normal Anatomy and Observations:
Regarding your observation about the visibility of blood vessels when you pull the scrotal folds flat, it is generally normal to see the veins of the scrotum. The scrotum contains a network of veins that can become more prominent due to various factors, including temperature changes, physical activity, or even the position of the body. However, if you notice any unusual swelling, changes in color, or if the veins appear engorged or painful, it would be advisable to consult a healthcare professional.

Recommendations:
Given your symptoms, it is important to seek a thorough evaluation by a healthcare provider, preferably a urologist. They may perform a physical examination and possibly an ultrasound to assess the blood flow and structure of the testicles and surrounding tissues. This will help determine the exact cause of your discomfort and guide appropriate treatment.

In summary, while varicocele is a possibility, other conditions could also be responsible for your symptoms. It is crucial to have a professional evaluation to rule out any serious issues and to receive appropriate care. Please do not hesitate to reach out to a healthcare provider for further assistance.
