Ejaculatory dysfunction?
Hello doctor, a few months ago after an episode of masturbation, I experienced difficulties with penile erection, sexual pleasure, and ejaculation.
During that session, just before climax, I practiced diaphragmatic breathing, inhaled deeply, and then tensed my abdomen, which caused the urge to ejaculate to disappear.
When I attempted to masturbate a second time, I did the same thing, which again made the urge to ejaculate vanish, but I did not feel any climax or ejaculation, and my penis became flaccid.
Does this mean I have already ejaculated? I felt like there was no semen released, and I did not experience the sensation of orgasm.
Could this indicate that I have caused damage to the internal structures or nerve conduction pathways, or is there an issue with the blood vessels? It has been several weeks, and doctors I have seen say it is unrelated to this behavior and suggest it is a psychological issue.
However, I am certain that everything changed after that episode of masturbation.
The condition of my penis has made me feel extremely distressed, to the point of contemplating suicide.
Please assist me, thank you.

Ejaculation difficulties can be a distressing issue that affects many men at some point in their lives. Understanding the potential causes and solutions is crucial for addressing this condition effectively. Based on your description, it seems that your difficulties began after a specific incident during masturbation, which may have triggered a psychological response that has since affected your sexual function.
Causes of Ejaculation Difficulties
1. Psychological Factors: Anxiety, stress, and depression can significantly impact sexual performance. The incident you described, where you consciously held your breath and tensed your abdomen, may have created a psychological block. This can lead to performance anxiety, where the fear of not being able to ejaculate or achieve orgasm becomes a self-fulfilling prophecy.
2. Physiological Factors: Although you mentioned that doctors have ruled out physical issues, it’s essential to consider that ejaculation difficulties can stem from various physiological causes, including nerve damage, hormonal imbalances, or vascular issues. Conditions such as diabetes, multiple sclerosis, or pelvic surgery can affect nerve function and blood flow, leading to difficulties in ejaculation.
3. Medication Side Effects: Certain medications, particularly antidepressants and antihypertensives, can interfere with sexual function. If you are on any medication, it’s worth discussing this with your healthcare provider.
4. Pelvic Floor Dysfunction: The muscles in the pelvic area play a crucial role in ejaculation. If these muscles are tense or not functioning correctly, it can lead to difficulties. This could be exacerbated by the way you held your breath and tensed your abdomen during the incident.
Solutions and Recommendations
1. Consult a Specialist: If you haven’t already, consider seeing a urologist or a sexual health specialist. They can perform a thorough evaluation, including physical examinations and possibly tests to rule out any underlying conditions.
2. Psychological Counseling: Since psychological factors seem to play a significant role in your situation, seeking help from a therapist who specializes in sexual health can be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and change negative thought patterns related to sexual performance.
3. Relaxation Techniques: Practicing relaxation techniques such as deep breathing, mindfulness, or yoga can help reduce anxiety and improve your overall sexual experience. Learning to relax your pelvic floor muscles can also be beneficial.
4. Gradual Exposure: If you feel comfortable, try gradually reintroducing sexual activity in a low-pressure environment. This could involve solo activities that focus on pleasure rather than performance, allowing you to reconnect with your body without the pressure of ejaculation.
5. Pelvic Floor Physical Therapy: A physical therapist specializing in pelvic floor dysfunction can help you learn how to relax and strengthen the pelvic muscles, which may improve your ability to ejaculate.
6. Medication Review: If you are taking medications, discuss with your doctor whether any of them could be contributing to your difficulties. There may be alternatives that do not have the same side effects.
7. Support Groups: Connecting with others who have experienced similar issues can provide emotional support and practical advice. Online forums or local support groups can be helpful.
Conclusion
Ejaculation difficulties can stem from a complex interplay of psychological and physiological factors. While it’s understandable to feel distressed about your situation, it’s essential to approach this issue holistically. Seeking professional help, whether through medical specialists or mental health professionals, can provide you with the tools and support needed to address these challenges effectively. Remember, you are not alone in this, and there are pathways to recovery and improvement.
