Teeth that have undergone root canal treatment may experience pain?

Hello, I have two lower molars that underwent root canal treatment but have not been crowned.
It has been over a year since the treatment, and recently, when I bite into harder foods (such as meat), I experience pain.
Could this be a reinfection by bacteria? If I want to get crowns, will I need to have another examination for root canal treatment? Thank you.

Niming, 30~39 year old female. Ask Date: 2022/03/08

Dr. Lin Jiayong reply Dentistry

Hello: It is recommended to first return to the original clinic where the root canal treatment was performed to confirm the recovery status after the treatment.
If a dentist has already suggested redoing the root canal, it would be advisable to undergo the retreatment before placing a crown.
Sincerely, Lin Chia-Yung, Department of Dentistry, Taichung Hospital, Ministry of Health and Welfare.

Tooth pain after root canal treatment can be a perplexing and distressing experience for many patients. Understanding the underlying causes of this pain and the appropriate solutions is crucial for effective management.

Firstly, it’s important to recognize that root canal treatment (RCT) is designed to remove infected or damaged pulp from the tooth, thereby alleviating pain and preserving the tooth structure. However, even after a successful RCT, patients may experience discomfort or pain for several reasons.

One common cause of pain after RCT is the presence of residual infection. If the root canal system is complex, with multiple canals or branches that are not adequately cleaned and disinfected, bacteria may remain. This can lead to reinfection, resulting in pain when biting down on hard foods. In your case, since you mentioned that the pain occurs specifically when chewing harder foods, it could indicate that there is still some inflammation or infection present in the root or surrounding tissues.

Another potential cause of pain is the development of periapical (root tip) inflammation. This condition can occur even after a root canal if the body’s immune response to bacteria is not fully resolved. The inflammation can lead to pressure buildup, causing pain when biting or chewing. Additionally, if the tooth has not been restored with a crown or other protective covering, it may be more susceptible to fractures or further damage, which can also result in pain.

Regarding your question about whether you need to undergo another root canal treatment before placing a crown, it is advisable to have a thorough examination by your dentist or an endodontist (a specialist in root canal treatments). They may recommend taking X-rays to assess the condition of the tooth and the surrounding bone. If there are signs of infection or if the root canal was not adequately filled, retreatment may be necessary to ensure that all infected tissue is removed and that the tooth is healthy enough to support a crown.

If the pain persists, your dentist may also consider other diagnostic tools, such as a cone beam computed tomography (CBCT) scan, which provides a more detailed view of the tooth’s anatomy and any potential issues that may not be visible on standard X-rays.

In terms of management, if reinfection is confirmed, your dentist will likely recommend retreatment of the root canal. This involves reopening the tooth, cleaning out the canals again, and then sealing them properly. After successful retreatment, a crown is usually recommended to protect the tooth from further damage and to restore its function.

In summary, tooth pain after root canal treatment can stem from residual infection, inflammation, or inadequate restoration. It is essential to consult with your dentist to evaluate the situation properly. They will guide you through the necessary steps, which may include further treatment or the placement of a crown, ensuring that your tooth is healthy and functional for years to come. Always remember that timely intervention can prevent more severe complications and preserve your oral health.
