Anal twitching?
Hello doctor, recently I have been feeling a pulsating sensation in my anus, with a frequency similar to that of a pecking chicken.
Could this be related to my emotions, or is there something wrong with my physical condition?
Mr. Huang, 30~39 year old female. Ask Date: 2022/03/14
Dr. Zhang Shuhao reply Surgery
This sensation of pulsation mostly originates from spasms of the pelvic floor muscles or the muscles around the anus.
It is advisable to try to relax, take a warm bath, and ensure adequate sleep.
If it does not affect your quality of life, no special treatment is necessary.
If there are concerns about other underlying conditions, a visit to a major hospital for an anorectal physiological examination may be warranted.

Pulsating sensations in the anus can be concerning and may arise from various causes. It's essential to understand that while some sensations might be benign, others could indicate underlying medical conditions that require attention.
One common cause of pulsating sensations in the anal region is muscle spasms. The anal sphincter is composed of smooth muscle, which can contract involuntarily due to various factors, including stress, anxiety, or even dietary choices. If you are experiencing heightened stress or anxiety, it is possible that these emotions could contribute to muscle tension and spasms, leading to the sensation you describe.
Another potential cause could be related to hemorrhoids. Hemorrhoids are swollen blood vessels in the rectal area that can cause discomfort, itching, and pulsating sensations. If you have a history of hemorrhoids or are experiencing other symptoms such as bleeding or pain during bowel movements, it would be wise to consult a healthcare provider for an evaluation.
Additionally, conditions such as anal fissures, which are small tears in the lining of the anus, can also lead to discomfort and unusual sensations. These fissures can occur due to straining during bowel movements or passing hard stools, and they may cause a sharp pain or a throbbing sensation in the anal area.
In some cases, nerve-related issues could be responsible for the sensations you are experiencing. Conditions such as pudendal neuralgia, which involves irritation or injury to the pudendal nerve, can lead to abnormal sensations in the anal region. This condition may be associated with prolonged sitting, pelvic trauma, or other factors.
It is also worth considering that certain gastrointestinal issues, such as irritable bowel syndrome (IBS) or inflammatory bowel disease (IBD), can lead to unusual sensations in the anal area. These conditions often come with other symptoms, such as changes in bowel habits, abdominal pain, or bloating.
If the pulsating sensation is persistent, worsening, or accompanied by other symptoms such as pain, bleeding, or changes in bowel habits, it is crucial to seek medical advice. A healthcare provider can perform a thorough examination, possibly including a digital rectal exam or other diagnostic tests, to determine the underlying cause and recommend appropriate treatment.
In summary, while pulsating sensations in the anus can be benign and related to muscle spasms or stress, they can also indicate more serious conditions such as hemorrhoids, anal fissures, or nerve-related issues. It is essential to monitor your symptoms and consult a healthcare professional if you have concerns, especially if the sensations persist or are accompanied by other troubling symptoms. Taking care of your mental health, managing stress, and maintaining a healthy diet can also contribute to overall well-being and may help alleviate some of the sensations you are experiencing.
