Left upper abdominal discomfort with pain upon pressure?

Doctor, I apologize for bothering you, but I would like to inquire about something.
Over the past few months, I have noticed a dull pain in the upper left abdomen, near the lower rib.
It hurts when I stand and when I press on it.
When I lie down on my stomach, especially on the left side, I can feel the pain quite clearly, but I don't feel anything when lying flat.
I found online that the upper left abdomen could be related to the pancreas, but I only feel pain in the front and not in my back.
Due to the pandemic, I wanted to ask you first what might be the issue.
Additionally, I have been dealing with chronic tonsillitis for the past few years (I had surgery last year) and have been on antibiotics and painkillers for about four years.
Recently, I also had my wisdom teeth extracted and underwent root canal treatment, during which I took antibiotics and painkillers.
I'm wondering if this could have affected my liver and if it is related to the tenderness I am experiencing.

Left upper abdominal pain can be a complex issue, as it may stem from various underlying conditions. Given your symptoms, which include pain near the rib cage that worsens with pressure and certain positions, it's essential to consider several possible causes.

One potential cause of left upper abdominal pain is related to the spleen, which is located in that area. Conditions such as splenomegaly (enlargement of the spleen) can lead to discomfort or pain, especially if there is any underlying infection or hematological disorder. Additionally, issues with the stomach, such as gastritis or peptic ulcers, can also present with pain in the upper abdomen, particularly after eating.

Another important consideration is the pancreas. Although you mentioned that you do not experience back pain, pancreatic issues can sometimes present with pain in the upper abdomen. Conditions such as pancreatitis or pancreatic cancer can cause discomfort in this region, although they are often accompanied by other symptoms like nausea, vomiting, or changes in appetite.

Your history of chronic tonsillitis and the use of antibiotics and painkillers raises another point of concern. Long-term use of certain medications can lead to gastrointestinal issues, including gastritis or even peptic ulcers, which could explain your symptoms. Furthermore, antibiotics can disrupt the normal gut flora, potentially leading to digestive issues that may manifest as abdominal pain.

Given that your pain is exacerbated by certain positions and is associated with meals, it may also be worth considering functional gastrointestinal disorders, such as gastritis or functional dyspepsia. These conditions can cause discomfort in the upper abdomen, particularly after eating, and may not always show up on imaging studies.

In terms of your concern about liver health, while long-term use of medications can have implications for liver function, it is essential to have liver function tests done to assess any potential damage. Symptoms of liver issues can sometimes overlap with abdominal pain, but they often include additional signs such as jaundice, dark urine, or changes in stool color.

Given the complexity of your symptoms and the duration of your discomfort, it is crucial to seek medical attention. A healthcare provider may recommend further diagnostic tests, such as blood tests to evaluate liver function, imaging studies like an abdominal ultrasound or CT scan, and possibly an endoscopy if gastrointestinal issues are suspected.

In summary, left upper abdominal pain can arise from various sources, including the spleen, stomach, pancreas, and even functional gastrointestinal disorders. Your history of medication use and the specific characteristics of your pain warrant a thorough evaluation by a healthcare professional to determine the underlying cause and appropriate treatment. It is advisable to consult with a gastroenterologist or your primary care physician to discuss your symptoms in detail and explore further diagnostic options.
