Teratoma and intermenstrual bleeding?
Hello doctor, last month during an abdominal ultrasound, a right ovarian cyst measuring 6-7 cm was detected.
My CA-125 level was normal, and the doctor initially diagnosed it as a teratoma.
No other abnormalities were found.
I plan to undergo surgery to remove the cyst at the end of the month.
My last menstrual period started on March 25 and ended on March 30, with light bleeding; by the third day, it was just brown discharge.
However, on April 4, I experienced vaginal bleeding again.
For the first two days, the amount was similar to my regular menstrual flow, without menstrual cramps but with a feeling of heaviness like during my period.
From April 6 to April 7, I only had blood when wiping during the day, but bleeding started again at night.
On April 5, I saw a doctor who prescribed Estrade and Fuan for nine days to regulate my menstrual cycle.
I found the side effects to be quite strong, so I haven't taken much of it.
I would like to ask: 1.
Could this intermenstrual bleeding or menstrual cycle irregularity be caused by the teratoma, or could it be due to other issues, since the abdominal ultrasound did not reveal any other abnormalities? 2.
What is the primary function of the medications I mentioned? If I continue taking them, can they help stop the bleeding? I'm concerned about the possibility of ongoing bleeding since I have laparoscopic surgery scheduled at the end of the month.
I apologize for the many questions, doctor!

Please follow the original doctor's instructions.

Understanding teratomas and their potential impact on abnormal vaginal bleeding is crucial for managing your health effectively. Teratomas are a type of germ cell tumor that can contain various types of tissues, such as hair, muscle, and even teeth. They are most commonly found in the ovaries in females and can vary in size, as you've noted with your 6-7 cm cyst. While teratomas are generally benign, they can sometimes lead to complications, including abnormal bleeding.
1. Abnormal Vaginal Bleeding and Teratomas: The connection between teratomas and abnormal vaginal bleeding is not straightforward. While teratomas themselves may not directly cause bleeding, they can lead to hormonal imbalances or other complications that might result in abnormal bleeding patterns. In your case, the presence of a teratoma could potentially disrupt the normal function of the ovaries, which may contribute to irregular menstrual cycles or unexpected bleeding. However, it is also essential to consider other factors that could be influencing your menstrual cycle, such as stress, changes in weight, or other underlying hormonal issues.
2. Other Potential Causes: Given that your CA-125 levels are normal and no other abnormalities were detected during the ultrasound, it is less likely that the teratoma is the sole cause of your bleeding. Other common causes of abnormal vaginal bleeding include hormonal imbalances, uterine fibroids, polyps, or even conditions like endometriosis. Since you are experiencing bleeding that resembles your menstrual cycle, it might also be related to your menstrual irregularities rather than the teratoma itself.
3. Medications Prescribed: Regarding the medications you mentioned—Estrade (which contains estradiol) and Fuan (which is likely a form of progesterone)—these are typically prescribed to help regulate menstrual cycles and manage symptoms associated with hormonal imbalances. Estradiol is a form of estrogen that can help thicken the uterine lining, while progesterone can help stabilize the lining and prevent excessive bleeding. If you are experiencing side effects from these medications, it is essential to discuss this with your healthcare provider. They may be able to adjust your dosage or suggest alternative treatments.
4. Managing Bleeding Before Surgery: If you are concerned about the bleeding continuing before your scheduled laparoscopic surgery, it is crucial to communicate this with your healthcare provider. They may recommend continuing the hormonal treatment to help manage the bleeding or suggest other interventions. It is also important to monitor the amount and duration of your bleeding, as significant changes could warrant further evaluation.
5. Surgical Considerations: As you prepare for surgery, your healthcare team will likely want to ensure that any bleeding is controlled to minimize complications during the procedure. If the bleeding persists or worsens, it may be necessary to reassess your treatment plan before the surgery.
In conclusion, while teratomas can be associated with abnormal bleeding, it is essential to consider a comprehensive approach that includes evaluating other potential causes. Your healthcare provider is your best resource for navigating these concerns, especially as you approach your surgery date. Open communication about your symptoms, treatment side effects, and any changes you experience will help ensure that you receive the most appropriate care.
