Sudden dizziness
Hello Dr.
Zheng, I suddenly experienced dizziness upon waking up yesterday (April 8), which was the first time I felt such severe dizziness that I was unable to stand.
Since I am currently abroad, I did not seek medical attention at that time.
I felt strange upon waking, so I took my temperature, which was around 38 degrees Celsius, and then the dizziness began.
The dizziness was not a spinning sensation; instead, my vision turned completely white, and my legs felt weak.
By the time I regained consciousness, I was already lying back in bed and sweating profusely.
Later, I checked my temperature again, and it had dropped to around 36.6 degrees Celsius.
I also took a COVID-19 rapid test, which was negative.
My medical history includes:
- After receiving the BNT vaccine, I experienced chest tightness, but all tests were normal.
- An echocardiogram indicated mild mitral and tricuspid valve regurgitation.
- Mild asthma.
- Palpitations (with a maximum heart rate of about 202 bpm while at rest; I often only sleep around 40 bpm).
I would like to ask:
1.
Why did I suddenly experience dizziness?
2.
How can I prevent future episodes of dizziness? I am particularly concerned since I live alone.
3.
What tests do you recommend?
4.
I have been feeling very fatigued; is this related to the dizziness?
5.
Is this related to my past medical history?

More Info
Dizziness can be a perplexing and concerning symptom, especially when it occurs suddenly and severely, as you described. Understanding the potential causes, preventive measures, and necessary medical evaluations is crucial for managing your health effectively.
1. Causes of Sudden Dizziness
Sudden dizziness can stem from various factors, including:
- Vestibular Disorders: Conditions affecting the inner ear, such as benign paroxysmal positional vertigo (BPPV), vestibular neuritis, or Meniere's disease, can lead to episodes of dizziness or vertigo.
- Cardiovascular Issues: Orthostatic hypotension (a drop in blood pressure when standing), arrhythmias, or other heart-related conditions can cause dizziness, especially if accompanied by symptoms like palpitations or chest discomfort.
- Neurological Conditions: Migraines, transient ischemic attacks (TIAs), or even strokes can present with dizziness as a symptom.
- Dehydration or Low Blood Sugar: Both can lead to dizziness, particularly if you have not eaten or hydrated adequately.
- Anxiety or Panic Attacks: Psychological factors can also manifest physically, leading to dizziness.
Given your recent symptoms, including elevated body temperature and cold sweats, it is essential to consider both infectious and non-infectious causes.
2. Preventing Future Dizziness Episodes
To prevent future episodes of dizziness, consider the following strategies:
- Stay Hydrated: Ensure you drink enough fluids, especially if you have been sweating or feeling unwell.
- Monitor Blood Pressure: If you experience dizziness upon standing, it may be beneficial to check your blood pressure regularly.
- Avoid Sudden Movements: When getting up from a lying or sitting position, do so slowly to prevent sudden drops in blood pressure.
- Manage Stress: If anxiety is a contributing factor, consider relaxation techniques such as deep breathing, meditation, or yoga.
- Regular Meals: Eating balanced meals at regular intervals can help maintain stable blood sugar levels.
3. Recommended Medical Evaluations
Given your symptoms and medical history, it is advisable to undergo several evaluations:
- Cardiac Evaluation: Given your history of palpitations and mild mitral regurgitation, a thorough cardiac assessment, including an electrocardiogram (ECG) and possibly a Holter monitor, may be warranted to rule out arrhythmias.
- Neurological Assessment: If dizziness persists, a consultation with a neurologist may be beneficial to rule out any central causes.
- Blood Tests: Checking for anemia, electrolyte imbalances, and thyroid function can provide insights into potential underlying issues.
- Imaging Studies: Depending on your physician's assessment, imaging studies such as an MRI or CT scan may be necessary to evaluate for any structural abnormalities.
4. Fatigue and Its Relation to Dizziness
Feeling fatigued can indeed be related to your dizziness. Dizziness can lead to increased fatigue due to the body's stress response and the energy expended in trying to maintain balance. Additionally, if you are not sleeping well due to anxiety or discomfort, this can exacerbate feelings of tiredness.
5. Connection to Past Medical History
Your previous medical history, including mild mitral regurgitation and episodes of chest discomfort, could be relevant. While these conditions may not directly cause dizziness, they can contribute to overall cardiovascular health, which is crucial in maintaining stable blood pressure and heart rhythm. It is essential to keep your healthcare provider informed about all your symptoms and medical history for a comprehensive evaluation.
Conclusion
In summary, sudden dizziness can arise from various causes, and understanding these can help in managing and preventing future episodes. It is crucial to seek medical advice, especially given your symptoms and medical history. Regular follow-ups and appropriate evaluations will aid in ensuring your health and well-being. If dizziness persists or worsens, do not hesitate to seek immediate medical attention.
