Pregnancy Chances: Signs and Symptoms After Ovulation - Obstetrics and Gynecology

Pregnancy probability

Hello Doctor, I would like to inquire about something.
My menstrual period started on March 24 and ended around March 30.
The period tracking app indicates that my ovulation day was April 8.
I had intercourse twice on April 5 and 6, both times with ejaculation.
Starting from April 7, I have been feeling a dull ache in my lower right abdomen, which has continued until today, April 9.
Is there a possibility that I could be pregnant?
I previously gave birth to a child on October 20, 2019.
It was quite challenging to conceive that child, and after realizing that I had difficulty with fertility, I did not engage in sexual activity again.
Since December 2020, I have not had a menstrual period for reasons unknown to me, and I have been seeing a doctor about it, but my period still did not return.
This continued for about seven months in a state of amenorrhea.
In August 2021, after stopping any medications or injections, my period suddenly returned and has been regular since then.
I still do not understand why I had no menstruation for those seven months.
😂
What I want to ask is, given my history of difficulty conceiving before pregnancy and the subsequent period of amenorrhea, I have recently started trying to conceive again.
Now, with this dull sensation in my abdomen over the past couple of days, is there a chance I could be pregnant? Is this a sign of pregnancy? (Because in the past, I rarely experienced ovulation pain and usually had no sensation.) Thank you for your help, Doctor.

Understanding the chances of pregnancy after ovulation involves recognizing various signs and symptoms that may indicate conception. Based on your timeline and symptoms, let's break down the situation.

You mentioned that your last menstrual period started on March 24 and ended around March 30, with an estimated ovulation date of April 8. You had intercourse on April 5 and 6, which is within the fertile window, as sperm can survive in the female reproductive tract for up to five days. The timing of your intercourse is favorable for conception, as it occurred just a few days before ovulation.

Starting from April 7, you began to feel a dull ache in your right lower abdomen, which you described as a persistent sensation. This type of discomfort can sometimes be associated with early pregnancy, but it can also be attributed to other factors, such as ovulation pain (mittelschmerz) or even premenstrual symptoms. Given that you typically do not experience ovulation pain, this sensation could be noteworthy.

In terms of pregnancy signs, some early indicators include:
1. Implantation Symptoms: If conception occurred, the fertilized egg would implant into the uterine lining about 6 to 10 days after ovulation. Some women experience light spotting or cramping during this time, which can be mistaken for a light period.

2. Breast Changes: Tenderness or swelling in the breasts can occur due to hormonal changes after conception.

3. Fatigue and Nausea: Many women report feeling unusually tired or experiencing nausea shortly after conception, although these symptoms typically develop a bit later.

4. Missed Period: The most definitive sign of pregnancy is a missed menstrual period. Since your cycle appears to be regular now, a missed period would be a strong indicator.

Given your history of difficulty conceiving and the irregularities you experienced previously, it is understandable to feel anxious about the possibility of pregnancy. The fact that you have resumed regular menstrual cycles since August 2021 is a positive sign, as it indicates that your body is functioning more normally.

To assess your chances of pregnancy more accurately, consider the following steps:
- Wait for a Missed Period: If your period does not arrive around the expected date, take a home pregnancy test. These tests are most accurate when taken after a missed period.

- Monitor Symptoms: Keep track of any additional symptoms you may experience in the coming days. If you notice significant changes, it may be worth consulting with your healthcare provider.

- Consult a Specialist: Given your history of irregular cycles and previous difficulties with conception, it may be beneficial to consult a fertility specialist. They can provide tailored advice and possibly run tests to assess your reproductive health.

In conclusion, while the symptoms you are experiencing could potentially indicate pregnancy, they are not definitive. The timing of your intercourse aligns well with your ovulation, which increases the chances of conception. However, the best course of action is to wait for a missed period and then take a pregnancy test for confirmation. If you have concerns about your fertility or any ongoing symptoms, do not hesitate to reach out to your healthcare provider for further evaluation and support.
