Bleeding issues

Hello doctor, I had laparoscopic surgery to remove an endometrioma at the end of March last year.
I received long-acting leuprolide injections in April and July of last year.
Starting in November, I began taking dienogest (I have been taking it regularly without missing any doses).
However, since I started taking dienogest, I have experienced brown discharge or light pink spotting for a few days each month, approximately the amount of a panty liner, about the size of a one-dollar coin.
Is this normal? I found online that cervical cancer can also cause brown bleeding.
I have never had sexual experience, and I went to a hot spring once (not sure if that counts as a source of infection).
I heard that a cervical exam is necessary, but since I have never had sexual experience, I am unsure what to do and am a bit worried about the bleeding.
I also don't know if this is due to the medication or another issue...
I'm not sure how to differentiate.

Abnormal bleeding after ovarian cyst surgery, particularly following the removal of a chocolate cyst, can be concerning for many patients. It is important to understand the potential causes of this bleeding, especially in the context of your medical history and the medications you are taking.

First, it is not uncommon for patients to experience some irregular bleeding or spotting after surgery, particularly when hormonal treatments like long-acting GnRH agonists (such as leuprolide, which you referred to as "長效達菲林") or hormonal contraceptives (like "異位寧") are involved. These medications can cause changes in the menstrual cycle, leading to breakthrough bleeding or spotting. The presence of brown discharge or light pink blood, as you described, can often be attributed to hormonal fluctuations and is generally considered normal, especially if it occurs sporadically and is not accompanied by other concerning symptoms.

However, it is crucial to differentiate between normal post-operative changes and symptoms that may indicate a more serious condition. The fact that you have not experienced any sexual activity reduces the likelihood of sexually transmitted infections, which can also cause abnormal bleeding. Nonetheless, it is essential to consider other potential causes, such as:
1. Hormonal Effects: As mentioned, hormonal treatments can lead to irregular bleeding. If you have been consistently taking your medications and have not missed doses, the bleeding could be a side effect of the hormonal therapy.

2. Cervical or Uterine Issues: While you mentioned concerns about cervical cancer, it is important to note that cervical cancer typically presents with more significant symptoms, such as heavy bleeding, unusual discharge, or pelvic pain. Regular gynecological exams, including Pap smears, are essential for monitoring cervical health, even if you have not been sexually active.

3. Infection: Although you mentioned a single visit to a hot spring, which is unlikely to be a significant risk factor for infection, any surgical procedure can carry a risk of infection. Symptoms of infection may include fever, increased pain, or foul-smelling discharge.

4. Residual Cyst or Complications: After surgery, it is possible for small cysts to remain or for new cysts to develop. If the bleeding persists or worsens, it may be worth investigating further with imaging studies or a follow-up appointment with your healthcare provider.

Given your concerns about the abnormal bleeding, it would be prudent to schedule a follow-up appointment with your gynecologist. They can perform a thorough examination, including a pelvic exam, to assess for any abnormalities. If necessary, they may recommend further testing, such as an ultrasound, to evaluate the status of your ovaries and uterus.

In summary, while some light spotting or brown discharge can be normal after ovarian cyst surgery and while on hormonal treatment, it is essential to monitor your symptoms closely. If you experience any additional symptoms such as heavy bleeding, severe pain, or signs of infection, seek medical attention promptly. Regular follow-ups with your healthcare provider are crucial to ensure your reproductive health is being adequately monitored and managed.
