Frequent urination issues?

Hello, Doctor.
On April 11, I started experiencing frequent urination after sexual intercourse, so I went to see a Western medicine doctor.
The urine test showed bacteria, and the doctor prescribed a 7-day course of antibiotics.
I only have frequent urination, with no pain or blood in the urine.
After finishing the antibiotics, I still feel the urge to urinate frequently, so on April 18, I decided to see a traditional Chinese medicine doctor.
They also conducted a urine test, and all values were normal with no bacteria present.
I find that I need to use the restroom approximately 10 minutes after drinking about 200cc of water, but I only feel the urge to urinate when I drink water.
At night, I usually sleep through until dawn (from around midnight to 6 AM).
During the day, I go to the restroom more than 10 times.
Could this situation indicate that the urinary tract infection is not completely resolved? The traditional Chinese medicine doctor prescribed medication targeting frequent urination, and I am also concerned about the possibility of overactive bladder syndrome.
I have experienced similar issues before, but they only occurred after sexual intercourse; when there is no sexual activity, I do not have frequent urination.

More Info

Frequent urination can be a distressing symptom that significantly impacts daily life and mental well-being. Understanding the underlying causes, potential treatments, and associated concerns is crucial for effective management.

Causes of Frequent Urination
Frequent urination can stem from various causes, including urinary tract infections (UTIs), bladder overactivity, prostate issues in men, and even psychological factors such as anxiety. In your case, the initial diagnosis of a UTI followed by antibiotic treatment suggests that an infection was likely contributing to your symptoms. However, the persistence of frequent urination after antibiotic therapy raises the possibility of other underlying conditions.

1. Urinary Tract Infection (UTI): UTIs are common causes of increased urinary frequency. They can lead to inflammation and irritation of the bladder, resulting in a frequent urge to urinate. If the infection was not fully resolved, it could explain your ongoing symptoms.

2. Bladder Overactivity: This condition, also known as overactive bladder (OAB), is characterized by a sudden urge to urinate that may be difficult to control. It can occur even in the absence of a UTI and may be triggered by various factors, including stress, certain foods, or bladder irritants.

3. Psychological Factors: Anxiety and stress can exacerbate urinary symptoms. The fear of needing to find a restroom can create a cycle of anxiety that further increases the urgency to urinate. This is particularly relevant if you have experienced anxiety related to your symptoms.

4. Other Medical Conditions: Conditions such as diabetes, interstitial cystitis, or prostate issues can also lead to frequent urination. It is essential to rule out these possibilities through appropriate medical evaluations.

Treatments for Frequent Urination
The treatment approach for frequent urination depends on the underlying cause. Here are some common strategies:
1. Antibiotics: If a UTI is confirmed, a course of antibiotics is typically prescribed. If symptoms persist after treatment, further evaluation may be necessary to ensure that the infection has been fully resolved.

2. Bladder Training: This involves gradually increasing the time between urinations to help retrain the bladder. It can be effective for those with overactive bladder symptoms.

3. Medications: Anticholinergic medications, such as Solifenacin, are often prescribed to help reduce bladder contractions and urgency. However, these medications can have side effects, so it’s essential to discuss any concerns with your healthcare provider.

4. Lifestyle Modifications: Reducing caffeine and alcohol intake, staying hydrated, and avoiding bladder irritants can help manage symptoms. Additionally, practicing relaxation techniques may alleviate anxiety related to urination.

5. Psychological Support: If anxiety is a significant factor, consulting with a mental health professional may be beneficial. Cognitive-behavioral therapy (CBT) can help address the psychological aspects of frequent urination.

Concerns and Next Steps
Given your ongoing symptoms, it is crucial to follow up with your healthcare provider. If the frequent urination persists despite treatment, further investigations may be warranted, including:
- Urodynamic Testing: This assesses how well the bladder and urethra are functioning.

- Cystoscopy: A procedure that allows direct visualization of the bladder to check for abnormalities.

- Blood Tests: To rule out conditions like diabetes or kidney issues.

In conclusion, frequent urination can be multifactorial, and a comprehensive approach is necessary for effective management. It is essential to communicate openly with your healthcare provider about your symptoms, treatment responses, and any psychological concerns you may have. This collaborative approach will help ensure that you receive the most appropriate care tailored to your specific needs.
