Swelling of the gums after wisdom tooth extraction?

Hello doctor, about 14 days ago I had my horizontal wisdom tooth extracted.
I have already had my stitches removed, and the extraction site has healed, but the gum below is still swollen.
When I lightly press on my cheek, some serous fluid seeps out.
During my last follow-up appointment after the stitches were removed, the doctor just mentioned that it is still swollen.
I have been on antibiotics for a long time, and I am hesitant to continue taking them.
Is it normal for the swelling to persist like this?

Dr. Zeng Guanghui reply Dentistry

The extraction site for the wisdom tooth is likely quite large.
Although the stitches have been removed, there is still a possibility of inflammation and infection at the wound site.
It is recommended that if symptoms persist, you should return to the dentist for an examination.

More Info

Swelling after wisdom tooth extraction is a common occurrence, but the extent and duration of the swelling can vary significantly from person to person. Generally, some degree of swelling is expected in the first few days following the procedure, peaking around the third day and then gradually subsiding. However, if you are experiencing persistent swelling two weeks post-extraction, especially if it is accompanied by other symptoms such as discharge of fluid, it may warrant further investigation.

The swelling you are experiencing could be due to several factors. One possibility is that it is part of the normal healing process. After a tooth extraction, the body responds to the trauma of the surgery with inflammation, which can manifest as swelling. This is part of the body's natural healing response, and while it can be uncomfortable, it is usually not a cause for alarm.

However, if the swelling persists beyond the typical healing timeframe, it could indicate a complication. One common issue that can arise after wisdom tooth extraction is an infection. Even with antibiotics, infections can sometimes develop, especially if there are remnants of tooth or bone left in the socket, or if food particles become trapped in the extraction site. Signs of infection may include increased pain, fever, or a foul taste in the mouth, in addition to swelling.

Another potential cause of prolonged swelling could be a condition known as dry socket (alveolar osteitis), which occurs when the blood clot that forms in the socket after extraction becomes dislodged or dissolves before the wound has fully healed. This can lead to significant pain and swelling, and it typically requires additional treatment from a dentist.

In your case, since you mentioned that there is a discharge of fluid when you apply pressure to your cheek, this could be a sign of an infection or another complication. It is important to communicate these symptoms to your dentist or oral surgeon. They may want to examine the extraction site to determine if there is an underlying issue that needs to be addressed.

Regarding your concern about continuing antibiotics, it is crucial to follow your dentist's advice. If you have completed the prescribed course and still have symptoms, it is advisable to return to your dentist for a follow-up. They may need to assess whether a different antibiotic is necessary or if other treatments are required.

In summary, while some swelling is normal after wisdom tooth extraction, persistent swelling accompanied by other symptoms such as discharge is not typical and should be evaluated by a dental professional. It is essential to maintain open communication with your healthcare provider about your symptoms and concerns to ensure proper healing and address any complications that may arise.
