The testicles feel firm?
Hello, Doctor.
At the end of January this year, I experienced a dull aching sensation in my scrotum, so I went to the hospital for a physical examination and ultrasound.
The doctor informed me that I have mild bilateral varicocele and that the intestinal wall on the left side is thinner.
Later, since I felt better, I did not continue to see the doctor.
However, this week I noticed that both of my testicles feel hard.
They are both uniformly hard, but their size has not significantly increased, and their shape appears normal without any particular protrusion on either side.
When I lie down to rest or wake up from sleep, I find that the testicles return to their original elastic state, and at that time, I do not feel any hard lumps; their shape and size are the same as before.
However, when I get up and engage in some daily activities, they become somewhat hard again.
I would like to know what might be causing this condition.
I read online that if it were testicular cancer, there should be irregular hard lumps, and it wouldn’t revert to the original state after sleeping.
Could this condition possibly be caused by varicocele or a hernia? I plan to see a doctor next week, but I am a bit anxious, so I wanted to ask for your advice first.
Thank you.

Hello: If the testicles are consistently hard, it is advisable to have a urology examination at the hospital.
If the hardness occurs intermittently, it is often related to vascular congestion, which can create a sensation of a lump.
Hernias are usually unilateral and rarely occur on both sides simultaneously.
Wishing you good health.

Understanding testicular hardness and its potential causes is crucial for maintaining reproductive health. Based on your description, it seems you are experiencing fluctuations in the firmness of your testicles, which can be concerning. Let's explore the possible reasons for this phenomenon and what you should consider moving forward.
Firstly, it’s important to understand that the testicles can change in firmness due to various factors, including temperature, physical activity, and underlying medical conditions. When you lie down or rest, the body temperature decreases, which can lead to the testicles becoming softer and more elastic. Conversely, during physical activity or when the body is warm, the testicles may feel firmer. This is a normal physiological response and is often not a cause for concern.
You mentioned that you have previously been diagnosed with mild varicocele (enlargement of the veins within the scrotum) and that you experienced a sensation of heaviness or discomfort. Varicoceles can lead to changes in testicular consistency and may contribute to feelings of discomfort or heaviness. They can also affect blood flow and temperature regulation in the testicles, which might explain the variations in firmness you are experiencing.
Regarding your concern about testicular cancer, it is true that testicular tumors typically present as hard, irregular masses that do not fluctuate in consistency like what you described. Testicular cancer often does not cause pain, and the presence of a hard mass that does not change with position or activity is a red flag. Since you mentioned that your testicles return to a normal state after resting, this is less indicative of a malignancy.
Another potential cause for the hardness could be related to the presence of a hernia. An inguinal hernia can sometimes cause a bulge or a feeling of firmness in the scrotal area, especially when standing or engaging in physical activities. However, hernias typically present with additional symptoms, such as pain or discomfort, particularly when lifting or straining.
Given your history of varicocele and the recent changes in testicular firmness, it is wise to consult with a healthcare provider. They may recommend a physical examination and possibly imaging studies, such as an ultrasound, to assess the blood flow and structure of the testicles and surrounding tissues. This will help rule out any serious conditions and provide peace of mind.
In summary, while fluctuations in testicular firmness can be normal, especially in the context of varicocele or other benign conditions, it is essential to monitor any persistent changes or new symptoms. Your plan to see a doctor next week is a prudent step. Make sure to communicate all your symptoms clearly, including the changes in firmness, any discomfort, and your medical history. This will help your healthcare provider make an accurate diagnosis and recommend appropriate treatment if necessary. Remember, early intervention is key to addressing any potential health issues effectively.
