I would like to inquire about the costs and differences between gastric bypass and gastric sleeve surgery?

Hello Doctor: I would like to ask about the main differences between gastric bypass and gastric sleeve surgeries, their respective costs, and the postoperative care considerations.
Currently, my height is 160 cm and my weight is 135 kg, giving me a BMI of 52.7, which qualifies as morbid obesity.
I have previously undergone gallbladder removal surgery.
I would like to know if I am still a suitable candidate for these surgeries or if further evaluations are necessary.
Thank you.

Jun, 20~29 year old female. Ask Date: 2003/08/07

Hello:
1.
According to the regulations of the National Health Insurance Administration, the implementation of gastric sleeve surgery must meet the following criteria: (1) dietary control for more than six months, (2) sound mental state, confirmed by a consultation with a psychiatrist with no abnormalities, (3) no other causes of morbid obesity due to metabolic or endocrine diseases, (4) age under 55 years, (5) ideal body weight exceeding 100% of the standard.
2.
The effects of gastric bypass surgery and gastric sleeve surgery are quite similar; however, gastric bypass surgery is more complex.
If one wishes to revert to the pre-surgical state, the surgery is more complicated.
3.
The cost of performing gastric sleeve surgery, due to the use of disposable laparoscopic instruments, is approximately NT$100,000.
4.
The mortality rate for both surgeries is about 5%.
Sincerely,
Dr.

When considering weight loss surgeries such as gastric bypass and sleeve gastrectomy, it's essential to understand the differences between these two procedures, their costs, and the postoperative care required. Both surgeries are effective for treating morbid obesity, but they have distinct mechanisms, risks, and recovery processes.

Differences Between Gastric Bypass and Sleeve Gastrectomy:
1. Procedure Type:
- Gastric Bypass (Roux-en-Y): This procedure involves creating a small pouch from the stomach and connecting it directly to the small intestine, bypassing a significant portion of the stomach and the first part of the small intestine. This not only restricts food intake but also alters the digestive process, leading to reduced calorie absorption.

- Sleeve Gastrectomy: In this procedure, approximately 75-80% of the stomach is removed, leaving a tube-like structure (sleeve). This significantly reduces the stomach's capacity, leading to decreased food intake and hormonal changes that promote weight loss.

2. Weight Loss Outcomes:
- Both surgeries can lead to significant weight loss, but gastric bypass may result in slightly more weight loss in the long term due to its malabsorptive component. However, sleeve gastrectomy is often favored for its simplicity and lower complication rates.

3. Risks and Complications:
- Both procedures carry risks such as infection, bleeding, and complications related to anesthesia. Gastric bypass has additional risks related to malabsorption, such as nutritional deficiencies (e.g., vitamin B12, iron, calcium) and dumping syndrome, where food moves too quickly from the stomach to the small intestine.

4. Recovery Time:
- Recovery times can vary, but generally, sleeve gastrectomy may have a shorter recovery period compared to gastric bypass. Patients can often return to normal activities within a few weeks after sleeve surgery, while gastric bypass may require a longer recovery.

Costs:
- The cost of these surgeries can vary widely based on geographic location, the hospital or surgical center, and whether the patient has insurance coverage. In general, sleeve gastrectomy may cost around $15,000 to $25,000, while gastric bypass can range from $20,000 to $30,000. It's essential to check with your healthcare provider and insurance company for specific costs and coverage options.

Postoperative Care:
- After either surgery, patients must adhere to a strict diet and follow-up care. This typically includes:
- A liquid diet for the first few weeks post-surgery, gradually transitioning to soft foods and then solid foods.

- Regular follow-ups with a healthcare provider to monitor weight loss, nutritional intake, and any potential complications.

- Lifelong vitamin and mineral supplementation, especially after gastric bypass, to prevent deficiencies.

Suitability for Surgery:
Given your height (160 cm) and weight (135 kg), your BMI of 52.7 indicates morbid obesity, which qualifies you for bariatric surgery. However, your previous gallbladder removal and any other medical conditions must be evaluated by a healthcare professional. It's crucial to undergo a comprehensive assessment, including a psychological evaluation, to ensure you are a suitable candidate for surgery. Factors such as age, overall health, and any comorbid conditions (like hypertension or heart disease) will influence the decision.

In conclusion, both gastric bypass and sleeve gastrectomy are effective options for weight loss in individuals with morbid obesity. The choice between the two should be made in consultation with a qualified bariatric surgeon who can assess your specific situation, discuss the potential risks and benefits, and help you determine the best course of action for your health and weight loss goals.
