Irregular menstruation?
Hello, my menstrual cycle usually varies within 2-3 days.
My last period started on March 28, and this time I had a small amount of bleeding last Wednesday (April 27).
It has been 5 days now without a full period, which has never happened before.
Usually, if I have a little spotting, my period would start by the next day at the latest.
What could be the possible reasons for this? On April 19, I was sitting with my boyfriend, and we were just hugging while wearing underwear, and he said he didn't ejaculate.
Can this factor be ruled out? Could stress be a possible cause?
Mo, 20~29 year old female. Ask Date: 2022/05/03
Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the factors that can affect the menstrual cycle or cause intermenstrual bleeding are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraceptive pills, diet, polycystic ovary syndrome, lesions of the uterus or cervix, infections, ovulation, pregnancy, and breastfeeding, among others.
It is advisable to consult a gynecologist if symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you peace and health, Dr.
Chang.

Irregular menstrual cycles can be a source of concern for many individuals, and understanding the potential causes is crucial for addressing any underlying issues. In your case, you mentioned that your menstrual cycle typically varies by only 2-3 days, but you experienced an unusual occurrence where you had a small amount of bleeding on April 27, and it has been five days without a full menstrual period. This deviation from your norm can be attributed to several factors.
Firstly, it’s essential to consider hormonal fluctuations. The menstrual cycle is regulated by a complex interplay of hormones, including estrogen and progesterone. Any disruption in this hormonal balance can lead to irregularities in your cycle. Factors such as stress, significant weight changes, or changes in physical activity can influence hormone levels. For instance, high levels of stress can lead to increased production of cortisol, which may inhibit the normal hormonal signals required for ovulation and menstruation.
In your case, you mentioned the possibility of stress as a contributing factor. Stress can indeed impact your menstrual cycle, leading to delayed periods or even skipped cycles. If you have been experiencing increased stress from work, personal relationships, or other life changes, this could be a significant factor in your irregular cycle.
Another consideration is the possibility of pregnancy, even if you believe the risk is low. While you indicated that you and your boyfriend were only in close contact and did not engage in penetrative intercourse, it’s important to note that sperm can sometimes be present in pre-ejaculate fluid. If there is any uncertainty, taking a home pregnancy test could provide clarity.
Additionally, other medical conditions can lead to irregular menstrual cycles. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or uterine fibroids can cause changes in menstrual patterns. If your irregularities persist or if you experience other symptoms such as severe pain, heavy bleeding, or significant mood changes, it would be advisable to consult a healthcare provider for a thorough evaluation.
In summary, irregular menstrual cycles can arise from various factors, including hormonal imbalances, stress, potential pregnancy, and underlying medical conditions. Given that this is a change from your typical pattern, it may be beneficial to monitor your cycle over the next few months. If irregularities continue or if you have additional concerns, seeking medical advice would be a prudent step. A healthcare provider can conduct necessary tests, including hormone level assessments, to determine the underlying cause and recommend appropriate treatment or lifestyle adjustments.
