The horizontal wisdom tooth has absorbed the adjacent molar issue?

Hello Doctor: Recently, I visited a clinic due to neuralgia, and after an X-ray, the physician discovered that I have a partially erupted horizontal wisdom tooth that has already absorbed half of the root of the molar, causing them to be fused together.
The doctor mentioned that my situation is quite unique and suggested treating the molar first before addressing the wisdom tooth.
I would like to ask the doctor: 1.
Is it possible to completely extract the wisdom tooth in this case? (Since it is already fused with the root of the molar) 2.
If the wisdom tooth is extracted, will the absence of the root cause instability in the molar? I am very concerned about the condition of my wisdom tooth.
Thank you for your clarification.

Hello: 1.
It is recommended to follow the physician's advice and treat the molars first before addressing the wisdom teeth.
If extraction is not feasible, consider leaving the wisdom teeth in place.
2.
Similarly, it is not advisable to sacrifice the anterior molars for the sake of extracting the wisdom teeth, unless there are other more serious and urgent reasons to do so!

Impacted wisdom teeth, also known as third molars, can indeed have significant effects on adjacent molars, particularly when they are positioned horizontally or are partially erupted. In your case, it sounds like the impacted wisdom tooth has caused resorption of the root of the adjacent molar, which is a serious condition that requires careful management.

1. Complete Extraction of the Wisdom Tooth: The extraction of an impacted wisdom tooth that has fused with the root of an adjacent molar can be complex. The degree of difficulty largely depends on the level of impaction and the relationship between the roots of the wisdom tooth and the adjacent molar. In some cases, the dentist or oral surgeon may need to remove the wisdom tooth in sections to minimize trauma to the surrounding structures. It is essential to have a thorough discussion with your oral surgeon about the specifics of your case, including the potential risks and the likelihood of complete removal.

2. Impact on the Adjacent Molar: If the wisdom tooth is fused with the root of the adjacent molar, there is a risk that the extraction could compromise the stability of the molar. If the extraction is not performed carefully, it could lead to further root resorption or even loss of the adjacent tooth. However, if the extraction is done by an experienced oral surgeon, they will take precautions to minimize damage to the adjacent tooth. It is also important to consider that if the wisdom tooth is causing problems, such as pain or infection, removing it may ultimately benefit the health of the adjacent molar.

In terms of your concerns about the stability of the adjacent molar after extraction, it is crucial to follow up with your dentist or oral surgeon after the procedure. They may recommend additional treatments, such as a dental crown or other restorative options, to ensure the long-term health and stability of the adjacent tooth.

Additional Considerations:
- Post-Operative Care: After the extraction, proper care is essential to promote healing and prevent complications. This includes following your dentist's instructions regarding pain management, diet, and oral hygiene.

- Monitoring: Regular follow-up appointments will be necessary to monitor the healing process and the condition of the adjacent molar. If any issues arise, such as increased mobility or pain in the molar, it is important to address them promptly.

- Long-Term Outlook: While the situation may seem daunting, many patients successfully undergo wisdom tooth extraction without long-term complications. The key is to work closely with your dental care team to ensure the best possible outcome.

In summary, impacted wisdom teeth can have significant implications for adjacent molars, particularly when they are fused. It is essential to have a comprehensive evaluation and treatment plan from an experienced dental professional. By addressing the issue proactively, you can help ensure the health and stability of your teeth moving forward.
