Vaginal bleeding after bowel movement?

Hello, I would like to ask about my situation.
I received a menstrual induction injection on April 19, and my period started on April 24.
Blood tests revealed that I have polycystic ovary syndrome (PCOS).
Starting from May 14, I have been experiencing vaginal bleeding during bowel movements.
I have confirmed that the bleeding is from the vagina, not the rectum.
I would like to know what this could be.
I have not engaged in sexual intercourse and do not have hemorrhoids.

Lele, 20~29 year old female. Ask Date: 2022/05/16

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, first determine whether it is the anus or the vagina.
If it is the anus, you should see a gastroenterologist.
If it is the vagina, monitor the situation for now.
If the same issue persists, it is advisable to consult a gynecologist.
Wishing you good health.

Vaginal bleeding after bowel movements can be concerning and may arise from various underlying causes. Given your situation, where you have a history of polycystic ovary syndrome (PCOS) and have recently undergone a menstrual induction injection, it is essential to consider several factors that could contribute to this bleeding.

Firstly, it is important to differentiate between vaginal bleeding and rectal bleeding. Since you have confirmed that the bleeding is from the vagina and not the rectum, we can focus on potential gynecological causes. Here are some possible explanations for vaginal bleeding during bowel movements:
1. Hormonal Fluctuations: After receiving a menstrual induction injection, your body may experience hormonal changes that can lead to irregular bleeding. This is particularly common in individuals with PCOS, as hormonal imbalances can cause unexpected bleeding episodes.

2. Cervical or Vaginal Trauma: Straining during bowel movements can sometimes lead to trauma in the vaginal or cervical area, especially if there is any underlying inflammation or sensitivity. This could result in minor bleeding.
3. Infections: Vaginal infections, such as bacterial vaginosis or sexually transmitted infections, can cause inflammation and bleeding. Even if you have not engaged in sexual activity recently, infections can occur without sexual contact.

4. Polycystic Ovary Syndrome (PCOS): Individuals with PCOS may experience irregular menstrual cycles and abnormal bleeding patterns. The hormonal imbalances associated with PCOS can lead to endometrial hyperplasia (thickening of the uterine lining), which may cause bleeding.

5. Endometrial or Cervical Issues: Conditions such as endometrial polyps, fibroids, or cervical lesions can lead to abnormal bleeding. These conditions may not always present with obvious symptoms until they cause bleeding or discomfort.

6. Vascular Issues: Sometimes, vascular malformations or varicosities in the pelvic region can lead to bleeding, particularly during activities that increase abdominal pressure, such as bowel movements.

Given your symptoms and medical history, it is crucial to seek medical attention. A healthcare provider can perform a thorough examination, which may include a pelvic exam, ultrasound, or other diagnostic tests to determine the exact cause of the bleeding.
In the meantime, it is advisable to monitor your symptoms closely. Note the amount and color of the blood, any associated pain, and any other symptoms you may experience. This information will be valuable for your healthcare provider in diagnosing the issue.

While it may be tempting to self-diagnose or treat with over-the-counter medications, it is essential to consult a healthcare professional. They can provide appropriate guidance and treatment options based on a comprehensive evaluation of your condition.

In conclusion, vaginal bleeding during bowel movements can stem from various causes, particularly in individuals with hormonal imbalances like PCOS. It is essential to seek medical advice to ensure proper diagnosis and management of your symptoms.
