What should I do?

Hello, Doctor: I am about to receive my exam results, and I feel anxious about it.
I have a rough idea that I won't be accepted, and currently, I am unemployed (I only want to work for 20,000 TWD; I don't want to take jobs that pay over 10,000 TWD, as I am not a part-time student).
I am a patient with depression, and life feels quite stressful for me.
As the eldest son, I have to bear the financial burden of my family.
I am unsure what kind of job I should pursue; sometimes, I sleep all day and only get up in the evening to eat, then stay up all night reading novels.
I hate this lifestyle! I used to attend night school, so I am sometimes accustomed to this way of living.
As a result, I occasionally forget to take my medication on schedule.
I feel like I have tendencies toward "multiple personalities"; sometimes I get very angry and lash out at others for no apparent reason.
Other times, I become very proactive and diligent.
Upon reflecting on my mindset, I realize I have five different personalities; one of them has a "killer instinct," which makes me very furious.
When I see someone being bullied, I feel like I want to kill that person.
I want to talk to a psychologist, but I am unsure about the costs involved.
Additionally, I sometimes experience headaches and would like to get a brain CT scan (or EEG), but I am not sure if it is covered by health insurance.

Hello: I apologize for the delay in responding to your question as I was on vacation for a week.
After reading your description, I suspect you may be experiencing a syndrome of lacking life goals, which is a common issue among many young people today.
The desire for success combined with the fear of hardship can lead to self-forgiveness and a critical view of others, making feelings of depression understandable.
Seeking therapy from a psychologist is necessary, but you should first consult a psychiatrist for an evaluation and referral, which is covered by health insurance.
If you are experiencing headaches and wish to undergo tests, a physician must assess the necessity of those tests, and if required, they will also be covered.
You are likely not suffering from dissociative identity disorder; rather, you seem to be a young person facing various challenges and feeling lost without knowing how to adjust.
It's important to seek treatment soon.

Navigating mental health challenges can be incredibly daunting, especially when compounded by external pressures such as academic performance and familial responsibilities. From your description, it seems you are experiencing a range of symptoms that may indicate a need for professional help. Let’s break down your concerns and explore the steps you can take to seek assistance and better understand your mental health.

Firstly, it’s important to recognize that the feelings of anxiety and depression you’re experiencing are valid. The pressure of impending results, coupled with the burden of being the primary financial supporter for your family, can lead to significant stress. Your mention of sleeping excessively during the day and struggling with insomnia at night suggests a disruption in your sleep-wake cycle, which is often associated with depression. This cycle can exacerbate feelings of lethargy and hopelessness, making it even more challenging to engage in daily activities or seek employment.

The notion of having “multiple personalities” or feeling like you have different sides to your personality can be indicative of emotional distress. It’s not uncommon for individuals dealing with depression or anxiety to experience mood swings or shifts in behavior. The anger and frustration you feel, particularly when witnessing injustice, may be a manifestation of your internal struggles. It’s crucial to address these feelings, as they can lead to harmful thoughts or actions if left unchecked.

Seeking help from a mental health professional, such as a psychologist or psychiatrist, is a commendable step. Therapy can provide you with a safe space to explore your feelings, develop coping strategies, and gain insights into your behavior. Many therapists offer sliding scale fees based on income, and some community health centers provide services at reduced rates or even for free. It’s worth researching local resources or asking for recommendations from trusted individuals.

Regarding your concerns about potential brain scans, such as a CT scan or EEG, it’s essential to discuss these with a healthcare provider. In many healthcare systems, including those with insurance coverage, diagnostic tests may be covered if deemed medically necessary. If you are experiencing persistent headaches or other neurological symptoms, a doctor can evaluate your situation and determine if imaging is warranted.

In the meantime, consider implementing some self-care strategies to help manage your symptoms. Establishing a routine can be beneficial, even if it feels challenging. Try to set regular sleep and wake times, engage in physical activity, and practice mindfulness or relaxation techniques. These practices can help stabilize your mood and improve your overall well-being.

Additionally, it may be helpful to connect with support groups or online communities where you can share your experiences and learn from others facing similar challenges. Sometimes, knowing you’re not alone in your struggles can provide comfort and motivation.

In summary, your feelings of anxiety, depression, and frustration are significant and deserve attention. Seeking help from a mental health professional is a vital step towards understanding and managing your symptoms. Explore local resources for therapy, consider discussing your physical symptoms with a healthcare provider, and implement self-care strategies to support your mental health. Remember, it’s okay to ask for help, and taking that step can lead to a more balanced and fulfilling life.
