Running a red light during menstruation?

Hello Doctor: I would like to ask, my girlfriend ran a red light during her menstrual period.
It was around the 4th day when she had almost no bleeding, and her cycle is about 20 to 22 days.
We used a condom throughout intercourse, and I checked it midway to ensure it was on properly.
However, at the end when I withdrew, there was a white substance at the vaginal opening that I couldn't determine if it was semen or discharge.
Afterward, I performed a water test on the condom, and there were no leaks.
Is it possible that the fluid came out from the back end of the condom? What is the likelihood of pregnancy in this situation?

Zhong, 20~29 year old female. Ask Date: 2022/05/24

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the questioner: For those with irregular menstrual cycles, ovulation may not necessarily occur early or on schedule.
The failure rate of contraception using condoms consistently is over 5%.
It is recommended to consider emergency contraception first, and if your period is more than a week late, you should still check for pregnancy.
Thank you! Wishing you peace and health.

When it comes to the question of whether you can get pregnant during your period, it is essential to understand the menstrual cycle and the factors that influence fertility. Generally, the menstrual cycle is divided into several phases: the menstrual phase, the follicular phase, ovulation, and the luteal phase. Ovulation typically occurs around the midpoint of the cycle, which is when a woman is most fertile. However, sperm can survive in the female reproductive tract for up to five days, which means that if you have unprotected sex towards the end of your period, there is a possibility of becoming pregnant if you ovulate shortly after.

In your case, you mentioned that your girlfriend's cycle is about 20 to 22 days long, and you had intercourse on the fourth day of her period when bleeding was minimal. While the chances of pregnancy during this time are generally lower, they are not zero. If she has a shorter cycle, ovulation could occur soon after her period ends, which increases the risk of sperm being present when the egg is released.

Regarding the use of condoms, it is crucial to ensure that they are used correctly to prevent any potential leakage. You mentioned that you checked the condom during intercourse and performed a water test afterward, which did not show any leaks. This is a good practice, as it helps confirm that the condom was intact. However, if there was any possibility that the condom slipped or broke, or if there was any semen that could have escaped from the back end of the condom, there is a slight risk of pregnancy.

The white substance you observed at the vaginal opening could be either semen or vaginal discharge. If it was semen and there was any leakage, it could potentially lead to pregnancy. However, if the condom was intact and used correctly, the likelihood of pregnancy is significantly reduced.

In summary, while the chances of getting pregnant during menstruation are generally low, they are not impossible, especially with a shorter cycle. If there are concerns about potential pregnancy, it is advisable for your girlfriend to take a pregnancy test if her next period is late. Additionally, if you are looking to avoid pregnancy in the future, consider discussing more reliable contraceptive methods with a healthcare provider.

It's also important to note that sexual health is a critical aspect of overall well-being. Open communication with your partner about contraception, sexual health, and any concerns can help both of you make informed decisions. If either of you has further questions or concerns about sexual health or pregnancy risks, consulting a healthcare professional is always a good idea. They can provide personalized advice based on individual circumstances and medical history.
