Testicular Injury: Can Atrophy Be Reversed? - Urology

Testicular injury

Hello Doctor, unfortunately, I have injured both of my testicles.
One side has epididymitis, and the other side has a bruise.
Both have shrunk and become softer for several years now.
However, my sexual function is normal; it's just that I feel particularly uncomfortable psychologically.
I would like to ask the doctor if testicular atrophy is truly irreversible.
Is there currently any method to make the shrunken testicles healthier or regain some size? I am willing to try any possible solutions.
Thank you for your guidance.

Hsu, 20~29 year old female. Ask Date: 2022/05/24

Dr. Du Shixi reply Urology

Hello: Generally, there won't be any changes after the developmental period.
Currently, it is advised to pay attention to circulation, which means wearing looser underwear and engaging in moderate exercise.
Wishing you good health.

Testicular atrophy, or the shrinkage of the testicles, can be a distressing condition for many men, particularly when it follows an injury. In your case, where both testicles have suffered trauma and one side has experienced inflammation of the epididymis, it is understandable that you are seeking answers regarding the potential for recovery and any available treatment options.

First, it is important to understand the underlying causes of testicular atrophy. This condition can result from various factors, including trauma, hormonal imbalances, infections, or vascular issues. In your situation, the trauma and subsequent inflammation may have led to damage in the testicular tissue, which can affect both size and function.

Regarding the reversibility of testicular atrophy, it is essential to note that while some cases of atrophy can be reversed, many instances, especially those resulting from significant trauma or prolonged inflammation, may lead to permanent changes in size and function. The testicles are sensitive organs, and once they have been damaged, the body may not always be able to restore them to their original state. However, this does not mean that all hope is lost.

Here are some potential avenues you might explore:
1. Hormonal Evaluation: It would be prudent to have your hormone levels evaluated, particularly testosterone and luteinizing hormone (LH). If there is a hormonal imbalance, hormone replacement therapy might be an option to consider, which could help improve testicular function and potentially stimulate some recovery in size.

2. Medical Treatment: Depending on the specifics of your injury and any ongoing inflammation, your doctor might recommend anti-inflammatory medications or other treatments to address any lingering issues. In some cases, medications that improve blood flow to the area may also be beneficial.

3. Surgery: In certain situations, surgical intervention may be warranted, especially if there is a structural issue or if the epididymis is causing ongoing problems. A urologist can provide guidance on whether surgery could be beneficial in your case.

4. Lifestyle Modifications: Maintaining a healthy lifestyle can also play a role in overall reproductive health. This includes regular exercise, a balanced diet, avoiding smoking and excessive alcohol consumption, and managing stress levels.

5. Psychological Support: Given your concerns about the psychological impact of testicular atrophy, it may be helpful to seek support from a mental health professional. They can provide strategies to cope with any anxiety or distress you may be experiencing related to your condition.

6. Regular Monitoring: Regular follow-ups with a healthcare provider specializing in male reproductive health can help monitor any changes in your condition and allow for timely interventions if necessary.

In summary, while testicular atrophy resulting from injury may not always be reversible, there are several potential avenues for treatment and management that could help improve your situation. It is crucial to work closely with a healthcare provider to explore these options and develop a personalized plan that addresses both the physical and psychological aspects of your condition. Remember, open communication with your healthcare team is key to finding the best approach for your unique circumstances.
