Emergency contraceptive pills & antibiotics

Hello, doctor.
Yesterday was the first day after my menstrual period.
After having unprotected intercourse, I took the emergency contraceptive pill Ella five hours later.
Today, I had a tooth extraction, and the dentist prescribed antibiotics (AMOXICILLIN) to be taken for three days.
Will taking this antibiotic affect the effectiveness of the emergency contraceptive pill? Is there any interaction between the medications, or how long should I wait before taking the antibiotic?

A Ying, 30~39 year old female. Ask Date: 2022/05/26

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, the two generally do not affect each other.
Wishing you good health.

When it comes to the interaction between antibiotics and emergency contraceptives, the situation can be quite nuanced. In your case, you mentioned taking Amoxicillin after having unprotected intercourse and subsequently taking emergency contraception (Ella). The primary concern is whether Amoxicillin will affect the effectiveness of the emergency contraceptive pill.

Generally, most antibiotics do not significantly impact the effectiveness of emergency contraceptives like Ella (ulipristal acetate) or Plan B (levonorgestrel). The primary antibiotics that have been shown to potentially interfere with hormonal contraceptives are rifampicin and rifabutin, which are used to treat tuberculosis and certain bacterial infections. These antibiotics can induce liver enzymes that metabolize hormones, potentially reducing the effectiveness of hormonal contraceptives. However, Amoxicillin is not one of those antibiotics.

Research indicates that Amoxicillin does not have a clinically significant effect on the efficacy of emergency contraceptives. Therefore, you can be reassured that taking Amoxicillin should not diminish the effectiveness of the emergency contraceptive you took.
As for the timing of taking the antibiotics, there is no need to wait or space out the doses of Amoxicillin and the emergency contraceptive. You can take them as prescribed by your healthcare provider.
However, it is always a good practice to monitor your body for any unusual symptoms or changes. If you experience any unexpected side effects or if your menstrual cycle does not return to normal after taking the emergency contraceptive, it would be wise to consult with your healthcare provider. They can provide further guidance and, if necessary, conduct a pregnancy test to rule out any possibilities.

In summary, taking Amoxicillin after taking Ella should not affect the effectiveness of the emergency contraceptive. You can continue with your prescribed antibiotic regimen without concern for interaction with the emergency contraceptive. Always keep open communication with your healthcare provider regarding any medications you are taking, especially when it comes to contraceptive methods and antibiotics.
