There is an unpleasant odor when exhaling through the nose?

Hello doctor, my family has noticed that over the past 1.2 years, especially when the weather gets colder (but not when it gets warmer), I exhale a strong odor from my nose.
I cannot smell it myself, but my family describes the smell as fishy.
This occurs about 3.5 times a year during the autumn and winter, and the odor lasts about 1 to 3 days before completely disappearing.
When I sleep, my husband has to use a fan to blow the smell out of the room, which is very bothersome.
I have no dental or oral issues, I am in good health, have a regular routine, and maintain a light diet.
I would like to ask which medical specialty would be more appropriate for treatment in an outpatient setting.
Thank you.

Hello, it is recommended to first consult an otolaryngologist for a nasopharyngoscopy to rule out conditions such as tumors.
Additionally, sinusitis, gastroesophageal reflux, gingivitis, and dental caries could also be sources of unusual odors.
Wishing you safety and good health.

Experiencing an unpleasant odor when exhaling through the nose can be a concerning symptom, especially when it is noted by family members but not perceived by the individual themselves. Given the details you've provided, it seems that this issue occurs primarily during colder weather and lasts for a few days at a time, which can be quite distressing for both you and your husband.
From a medical perspective, there are several potential causes for the odor you are experiencing. One common reason could be related to sinus issues, such as sinusitis or nasal polyps, which can lead to the accumulation of mucus that may harbor bacteria, resulting in a foul smell. Another possibility could be related to post-nasal drip, where mucus drips down the back of the throat, potentially leading to an unpleasant odor when exhaled. Additionally, conditions such as halitosis (bad breath) can sometimes manifest as an odor when exhaling through the nose, even if oral hygiene is maintained.

Given that you have no dental issues and are otherwise healthy, it would be prudent to consult an Ear, Nose, and Throat (ENT) specialist. An ENT doctor can conduct a thorough examination of your nasal passages and sinuses, possibly using endoscopy to visualize any abnormalities that may not be apparent through a standard examination. They can also assess for signs of infection or inflammation that could be contributing to the odor.

In terms of treatment, the ENT specialist may recommend various approaches depending on the underlying cause. If sinusitis is diagnosed, treatment may include nasal corticosteroids, saline nasal rinses, or antibiotics if a bacterial infection is suspected. If nasal polyps are found, they may suggest surgical options to remove them.
It's also worth noting that environmental factors, such as exposure to certain allergens or irritants, could exacerbate your symptoms, particularly in colder weather when indoor heating can dry out nasal passages. Maintaining good hydration and using a humidifier during the winter months may help alleviate some symptoms.

In summary, based on the symptoms you've described, it is advisable to seek an evaluation from an ENT specialist. They will be able to provide a comprehensive assessment and appropriate treatment options tailored to your specific situation. Additionally, keeping a symptom diary, noting when the odor occurs and any associated factors, could be beneficial for your consultation. This information can help the doctor in diagnosing the issue more effectively.
