Eye problems
Hello, doctor.
I would like to ask you about a symptom I experience.
Every morning, the moment I open my eyes, I feel like the images I see are vibrating, especially in the upper part of my vision.
However, after blinking a few times, it quickly resolves.
When I get up at night to use the bathroom, I don't notice this sensation, possibly because it's darker.
I suspect it might be nystagmus, as I feel that my eyelids are not moving.
Could you please tell me what might be wrong with my eyes? Thank you.

Dr. Liang Ce reply Ophthalmology

Experiencing visual disturbances, such as vibrations or shaking of images upon waking, can be concerning. However, it is essential to understand that these sensations are often benign and may be related to various factors, particularly when they resolve quickly after blinking or adjusting your eyes.
Possible Causes of Morning Eye Vibrations
1. Transient Visual Disturbances: Upon waking, your eyes may still be adjusting from a state of rest to alertness. This adjustment can lead to temporary visual disturbances, including the sensation of vibrations or shaking images. This phenomenon is often due to the brain and eyes recalibrating after a period of inactivity during sleep.
2. Dry Eyes: If you have a history of dry eyes, which is common among individuals with conditions like dry eye syndrome or prolonged screen time, your eyes may not produce enough tears during the night. Upon waking, the lack of moisture can lead to blurred vision or a sensation of vibrations until your eyes are adequately lubricated through blinking.
3. Eye Fatigue: If you have been using your eyes extensively before sleeping, such as reading or using screens, your eyes may be fatigued. This fatigue can manifest as visual disturbances upon waking. The muscles controlling eye movement may need a moment to regain their coordination after a period of rest.
4. Visual Snow Syndrome: Although less common, some individuals experience a condition known as visual snow, where they see persistent visual disturbances, including flickering or vibrating images. This condition is typically not associated with any underlying eye disease but can be distressing.
5. Neurological Factors: In some cases, visual disturbances can be linked to neurological conditions. However, if these sensations are transient and resolve quickly, they are less likely to indicate a serious issue.
Solutions and Recommendations
1. Hydration and Eye Care: Ensure that you are adequately hydrated and consider using artificial tears before bed to help maintain moisture in your eyes overnight. This can reduce the likelihood of dry eyes upon waking.
2. Sleep Hygiene: Maintain good sleep hygiene by ensuring you get adequate rest and avoiding excessive screen time before bed. This can help reduce eye fatigue and improve overall eye health.
3. Blinking Exercises: Incorporate blinking exercises into your routine, especially after prolonged screen use. This can help stimulate tear production and keep your eyes lubricated.
4. Consult an Eye Specialist: If the sensations persist or worsen, it is advisable to consult an eye specialist. They can perform a comprehensive eye examination to rule out any underlying conditions and provide tailored advice based on your specific symptoms.
5. Regular Eye Check-ups: Since you mentioned having regular eye examinations, continue this practice. Regular check-ups can help monitor your eye health and catch any potential issues early.
Conclusion
In summary, the sensation of visual vibrations upon waking is often a temporary and benign issue related to eye adjustment, dryness, or fatigue. While it is generally not a cause for alarm, maintaining good eye care practices and consulting with an eye care professional if symptoms persist is essential. Your proactive approach to monitoring your eye health is commendable, and regular check-ups will help ensure that any potential issues are addressed promptly.
