Are you pregnant?

Hello doctor, I would like to ask if on May 14 and June 1, I engaged in fondling and genital rubbing with my girlfriend (without using a condom) and ejaculated (on her stomach or hand) without penetration.
After each ejaculation, I would clean up with tissue and continue rubbing (with my fingers inserted).
She mentioned that her ovulation period was on May 16.
Her menstrual cycle is irregular, and she is now 10 days late (including today).
She is still a virgin.
After each ejaculation, I continued to rub for about five times.
Doctor, could you please tell me if this is due to pregnancy causing her missed period, or could it be due to hormonal imbalance or stress? Thank you, doctor.

Niming, 10~19 year old female. Ask Date: 2022/06/10

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the factors that can affect the menstrual cycle are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraception, diet, polycystic ovary syndrome, lesions in the uterus or cervix, infections, ovulation, pregnancy, and breastfeeding, among others.
It is recommended that if your normal menstrual period does not arrive, you should consult a gynecologist for evaluation.
Thank you! Wishing you peace and health, Dr.
Chang.

When considering the possibility of pregnancy, especially in the context of irregular menstrual cycles, it is essential to understand the various factors that can influence both menstruation and conception. In your case, the timeline of events and the nature of the sexual activity described raises several important points to consider.

Firstly, the timing of ovulation is crucial. You mentioned that your girlfriend indicated her ovulation occurred around May 16. Sperm can survive in the female reproductive tract for up to five days, which means that sexual activity occurring on May 14 could potentially lead to pregnancy if ovulation occurred shortly thereafter. The fact that there was ejaculation, even if not directly inside the vagina, raises the possibility of sperm being present in the vaginal area, which could also lead to conception.

Regarding the missed period, it is important to note that stress, hormonal imbalances, and changes in routine can all contribute to irregular menstrual cycles. If your girlfriend has a history of irregular periods, it is possible that her cycle may be influenced by factors such as stress or changes in lifestyle. However, a missed period can also be one of the earliest signs of pregnancy, especially if it is accompanied by other symptoms such as nausea, breast tenderness, or fatigue.

Given that her period is already ten days late, it would be advisable for her to take a home pregnancy test. These tests are designed to detect the hormone hCG (human chorionic gonadotropin), which is produced shortly after a fertilized egg attaches to the uterine lining. If the test is positive, it would confirm pregnancy, and she should schedule an appointment with a healthcare provider for further evaluation and prenatal care.

If the test is negative and her period does not arrive, it may be beneficial for her to consult with a healthcare professional. They can assess her overall health, review her menstrual history, and possibly conduct further tests to determine if there are underlying issues such as hormonal imbalances or other medical conditions that could be affecting her menstrual cycle.

In summary, while the possibility of pregnancy exists given the timing and nature of the sexual activity, other factors such as stress and hormonal fluctuations could also be contributing to the missed period. A home pregnancy test is a straightforward first step to clarify the situation, followed by a consultation with a healthcare provider for further guidance and support.
