Questions about emergency contraceptive pills and the use of oral contraceptives?

Hello, Doctor: Recently, my menstrual cycle has been irregular, so I visited a gynecologist and received the oral contraceptive pill, Alesse, to regulate my cycle and for contraception.
After finishing one month, I took a 7-day break before starting the next pack.
However, after taking 7 pills from the second pack, I still experienced some light bleeding.
I went back to the gynecologist for an examination, and the doctor noted that my endometrium was thicker than normal, so they prescribed me Metformin to take alongside Alesse.
Yesterday was my first day taking both medications together.
I would like to know if this will affect the contraceptive effectiveness, or if I need to consider additional contraceptive measures.

K, 20~29 year old female. Ask Date: 2022/06/10

Dr. Cao Guotao reply Obstetrics and Gynecology

Combining birth control pills can indeed affect their effectiveness, but it largely depends on the specific medications involved and how they interact with each other. In your case, you are taking a combination of a birth control pill (Alesse) and another medication (likely a progestin or another hormonal medication) prescribed to address your thickened endometrium.
First, it’s important to understand how birth control pills work. They primarily function by preventing ovulation, thickening cervical mucus to block sperm, and thinning the uterine lining to prevent implantation. When you introduce another medication, it can potentially alter these effects.
In general, if both medications are hormonal and prescribed by a healthcare provider, they are typically designed to work together. However, there are a few considerations to keep in mind:
1. Drug Interactions: Some medications can reduce the effectiveness of birth control pills. For example, certain antibiotics, anti-seizure medications, and herbal supplements like St. John’s Wort can interfere with hormonal contraceptives. It’s essential to check with your healthcare provider or pharmacist about any potential interactions between Alesse and the new medication you are taking.

2. Timing and Consistency: The effectiveness of birth control pills can also be influenced by how consistently and correctly they are taken. If you miss a dose or take them at irregular times, it may increase the risk of unintended pregnancy. Since you mentioned experiencing some breakthrough bleeding, it’s crucial to ensure that you are taking both medications as directed.

3. Breakthrough Bleeding: The presence of breakthrough bleeding while on birth control can be common, especially when starting a new medication or if the body is adjusting to hormonal changes. This does not necessarily indicate that the birth control is ineffective, but it can be a sign that your body is adapting to the hormonal changes.

4. Additional Contraceptive Measures: If there is any doubt about the effectiveness of your birth control regimen, it is wise to use additional contraceptive methods, such as condoms, until you are sure that the combination of medications is working effectively. This is particularly important if you are concerned about the risk of pregnancy.

5. Follow-Up with Your Doctor: Since you are under the care of a healthcare provider, it’s essential to maintain open communication with them. If you have concerns about the effectiveness of your birth control or experience any unusual symptoms, do not hesitate to reach out for further evaluation and guidance.

In summary, while combining Alesse with another medication can potentially affect its effectiveness, it largely depends on the nature of the second medication and how it interacts with the birth control pill. Always consult with your healthcare provider for personalized advice and to ensure that you are using the most effective contraceptive method for your situation. If you have any concerns about breakthrough bleeding or the effectiveness of your birth control, it’s best to discuss these with your doctor, who can provide tailored advice based on your health history and current medications.
