Left chest tightness and pain?


Hello, doctor! I've been experiencing left chest tightness and pain for the past two days.
Sometimes the pain is closer to the nipple, but there is no pain on the right side.
The tightness persists throughout the day.
My menstrual period just ended a few days ago, and I'm wondering if this could be related or if there are other factors involved.

When a clinician addresses the issue of chest pain, the first four critical conditions to rule out are myocardial infarction, pulmonary embolism, pneumothorax, and aortic dissection.
If none of these conditions are present, the differential diagnosis should consider lung tumors, lung abscesses, pneumonia, tuberculosis, arrhythmias, congenital heart disease, myocardial ischemia, pericardial issues, gastroesophageal reflux disease, esophageal tumors, esophageal structural abnormalities, musculoskeletal problems, neuritis, breast diseases, and so on.
Additionally, anxiety disorders, hypochondriasis, and psychoneurosis should also be considered.

If menstrual-related issues are taken into account, one must also consider primary dysmenorrhea and secondary dysmenorrhea.
Primary dysmenorrhea is primarily associated with the secretion of "prostaglandins" (prostaglandin F2α, prostaglandin E2), "leukotrienes," "platelet activating factor," and "vasopressin." Prostaglandins can increase the frequency and intensity of uterine contractions, leading to excessive contractions, vascular compression, and subsequent ischemia and hypoxia of the uterus, which can cause back pain and lower abdominal pain.
This typically occurs 1-2 hours before or after menstruation and is seen within the first 1-2 years after menarche, lasting less than 72 hours each time.
Secondary dysmenorrhea refers to menstrual pain caused by diseases or abnormalities of the reproductive system, such as ovarian cysts, endometriosis, fibroids, uterine displacement, and infections of the reproductive organs.
It usually appears a few years after menarche, with a sensation of pelvic heaviness and back pain gradually increasing during the late luteal phase, peaking at the onset of menstruation.
It is often accompanied by irregular menstrual cycles, heavy menstrual bleeding, dyspareunia, or increased vaginal discharge.
