Is it safe to consume Sanqi (Notoginseng) and Danggui Shaoyao San during menstruation?
Hello Doctor, I have been experiencing irregular menstruation (my period has lasted for nearly four months this time, and previously due to the COVID vaccine, I had my first dose followed by a month, and the second dose two months later), along with insomnia (waking up around three or four in the morning with dry mouth).
I consulted a traditional Chinese medicine practitioner (Western medicine only found thickening of the endometrium).
The doctor prescribed Danggui Shao Yao San, Sanqi, and Zhibai Dihuang Wan.
However, after taking them for two days, my menstruation started again.
Is it appropriate to continue taking this herbal formula during my menstrual period? Thank you for your response!

Dr. Gao Peiyi reply Traditional Chinese Medicine
Danggui Shaoyao San, Zhibai Dihuang Wan, and their relation to menstrual conditions are not significant.
There is some controversy regarding Sanqi; if the daily dosage of this herbal medicine exceeds one gram, it is recommended to discontinue use.
However, the impact of medications accounts for only a small portion of the issue, with the primary cause being endometrial thickening.
Wishing you a speedy recovery.

When considering the use of traditional Chinese medicine (TCM) formulas like Sanqi (Panax notoginseng) and Danggui Shaoyao San (a formula containing Angelica sinensis and Paeonia lactiflora) during menstruation, it is essential to understand both the properties of these herbs and the physiological changes that occur during the menstrual cycle.
Sanqi is known for its ability to invigorate blood circulation and reduce swelling. It is often used to address issues related to blood stasis, which can manifest as pain or discomfort. Danggui Shaoyao San, on the other hand, is traditionally used to nourish the blood and alleviate pain, particularly in the context of menstrual discomfort. It is commonly prescribed for conditions related to blood deficiency and is thought to help regulate menstrual cycles.
In TCM, menstruation is viewed as a time when the body undergoes significant changes. The menstrual phase is characterized by the shedding of the uterine lining and can involve a loss of blood and energy. Therefore, the use of certain herbs during this time can be a point of contention. Some practitioners may advise against using blood-invigorating herbs like Sanqi during menstruation, as they could potentially exacerbate bleeding or cause discomfort. Conversely, herbs that nourish the blood, like those found in Danggui Shaoyao San, may be considered beneficial, especially if there is a concern about blood deficiency.
Given your situation, where you are experiencing prolonged menstrual bleeding (menorrhagia) and have been diagnosed with thickened endometrial lining, it is crucial to approach the use of these herbs with caution. While Danggui Shaoyao San may help in nourishing the blood and alleviating some menstrual symptoms, the potential effects of Sanqi during menstruation should be carefully considered.
It is advisable to consult with a qualified healthcare professional, preferably one who specializes in TCM, to assess your specific condition and determine the best course of action. They can provide personalized guidance based on your symptoms, medical history, and the current state of your menstrual cycle. Additionally, if you are experiencing significant discomfort or changes in your menstrual pattern, it may be worth revisiting your Western medical provider to rule out any underlying conditions that may require further investigation or treatment.
In summary, while Danggui Shaoyao San may be beneficial during menstruation, the use of Sanqi should be approached with caution. Always consult with a healthcare professional before continuing or altering any treatment regimen, especially during sensitive times like menstruation. This ensures that you are taking the safest and most effective approach to managing your health.
