Post-abortion surgical conditions?

Hello Dr.
Lu: This is my first pregnancy, and I am over two months along.
However, the baby has not grown, and I have experienced bleeding along with blood clots.
The doctor diagnosed it as tissue from the baby, so we chose to undergo a dilation and curettage (D&C) procedure.
I would like to ask if it is normal to have bleeding for one to three days after the surgery, and whether my menstrual period will start right away.
I just had the surgery, and the bleeding seemed to almost stop the next morning, but then it increased again in the afternoon.
I have been bleeding for four days, and it seems to be tapering off on the fifth day.
People have told me that this could be my period starting, as I usually experience cramps when my period begins.
Is this normal? Additionally, I would like to know how long after the surgery it is safe to resume sexual intercourse with my husband without affecting my health, as he is concerned about hurting me.
Thank you for your response, Dr.
Lu...^-^

Hello! 1.
After undergoing a dilation and curettage (D&C) procedure, it is common to experience bleeding for 2-3 days, which should gradually decrease.
However, if you are experiencing additional bleeding that is not menstrual, possible reasons may include retained placental tissue, poor uterine contractions, or uterine inflammation, among others.
Therefore, it is generally recommended to return for a follow-up appointment one week after the procedure to confirm that there are no issues.
2.
Typically, as long as there are no problems during the follow-up visit one week after surgery, you can resume sexual activity.
Best wishes for your health from Dr.

More Info

After undergoing a surgical procedure such as a dilation and curettage (D&C) following a miscarriage or abortion, it is essential to understand the recovery process and what to expect in terms of bleeding and resuming intimacy.
Firstly, it is common to experience some bleeding after a D&C. This bleeding can vary in intensity and duration. Generally, patients may experience light bleeding for a few days post-surgery, which can gradually taper off. However, it is not unusual for bleeding to fluctuate, with some days being heavier than others. The bleeding you described, where it seemed to stop and then increase again, is not uncommon. This can be due to the body’s natural healing process as the uterus sheds any remaining tissue.
Typically, after a D&C, it may take a few days to a week for the bleeding to subside. If you find that the bleeding is heavy (soaking through a pad in an hour or less) or accompanied by severe pain, fever, or foul-smelling discharge, it is crucial to contact your healthcare provider. These could be signs of complications such as infection or retained tissue.

Regarding the return of your menstrual cycle, it is important to note that it may not resume immediately after the procedure. Many women find that their periods can take some time to return to their usual cycle, and it may be a few weeks to a couple of months before you have your first period post-D&C. The bleeding you are experiencing now may not necessarily be your menstrual period but rather the body’s response to the procedure. If you have concerns about your menstrual cycle or if it does not return within a reasonable timeframe, a follow-up appointment with your healthcare provider is advisable.

As for resuming sexual activity, it is generally recommended to wait at least two weeks after a D&C before engaging in intercourse. This waiting period allows your body to heal and reduces the risk of infection. However, it is essential to listen to your body and consult with your healthcare provider for personalized advice. If you feel any discomfort or have concerns about resuming intimacy, it is best to discuss these with your doctor.

In summary, while some bleeding after a D&C is normal, it is essential to monitor its intensity and duration. Your menstrual cycle may take some time to normalize, and it is advisable to wait at least two weeks before resuming sexual activity. Always prioritize your health and well-being, and do not hesitate to reach out to your healthcare provider with any questions or concerns during your recovery process.
