Uncertain whether it's ear pain, wisdom tooth pain, or something else?

Hello Doctor, I have experienced the following conditions several times: 1.
The pain is not brief; it can last for several hours.
2.
Applying pressure to the area around the ear reduces the pain.
3.
Occasionally, I have had brief episodes of sharp pain in my right ear or a buzzing sound.
I previously had issues with a cavity in my upper right wisdom tooth, and I'm uncertain about the source of the problem.
Thank you.

Tai, 20~29 year old female. Ask Date: 2022/06/20

Dr. Li Jiaxuan reply Otolaryngology

The Eustachian tube, inner ear, and temporomandibular joint may all warrant a recommendation for outpatient consultation.
Wishing you good health.

Based on the symptoms you've described, it seems that you are experiencing a complex interplay of potential issues involving both your ear and your wisdom teeth. Let's break down the possibilities and provide a clearer understanding of what might be happening.

1. Ear Pain: The pain you are experiencing in your ear could be attributed to several factors. Given that you mentioned the pain lasts for several hours and is alleviated by applying pressure to the area around your ear, this could indicate a few things. One possibility is that you may be experiencing eustachian tube dysfunction or acute otitis media, which is an infection of the middle ear. This can cause significant pain and may also lead to a sensation of fullness or pressure in the ear. The "buzzing" sound you mentioned could be a form of tinnitus, which often accompanies ear issues.

2. Wisdom Tooth Pain: The mention of your upper right wisdom tooth being decayed raises the possibility that the pain could also be referred pain from dental issues. Wisdom teeth, especially when they are impacted or decayed, can cause significant discomfort that radiates to the ear, jaw, and even the temples. This is due to the close anatomical relationship between the teeth and the nerves that supply sensation to the ear and jaw. If the wisdom tooth is infected or causing inflammation in the surrounding tissues, it could lead to pain that mimics ear pain.

3. Temporomandibular Joint (TMJ) Disorders: Another potential cause of your symptoms could be related to the temporomandibular joint, which connects your jaw to your skull. TMJ disorders can cause pain in the jaw, ear, and even headaches. If you have been grinding your teeth or clenching your jaw (which can happen during times of stress), this could lead to inflammation and pain in the joint, which may also be felt in the ear area.

4. Sinus Issues: Sinus infections or sinusitis can also lead to ear pain. The sinuses are located near the ear, and inflammation or infection in the sinuses can create pressure that is felt in the ear. If you have had any recent upper respiratory infections or allergies, this could be a contributing factor.

Given the complexity of your symptoms, it is crucial to seek a comprehensive evaluation from a healthcare professional. A thorough examination, including a dental evaluation and possibly imaging studies (like X-rays), may be necessary to determine the exact cause of your pain.
In the meantime, here are some general recommendations:
- Pain Management: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help alleviate discomfort. However, consult with a healthcare provider before starting any medication.

- Warm Compress: Applying a warm compress to the affected ear or jaw area may provide some relief from pain.

- Avoid Irritants: If you suspect that allergies or sinus issues may be contributing to your symptoms, try to avoid known allergens and consider using saline nasal sprays to help clear your sinuses.

- Dental Consultation: Given your history of wisdom tooth issues, it would be wise to consult a dentist to evaluate the condition of your wisdom teeth and determine if they require treatment.

In conclusion, while your symptoms could stem from ear pain, wisdom tooth issues, or even TMJ disorders, a thorough evaluation by a healthcare professional is essential to pinpoint the exact cause and provide appropriate treatment.
