Chest tightness with some difficulty breathing, relieved after belching?

Hello doctor, I went out for errands at 2:30 PM the day before yesterday and was exposed to the sun for about 20 minutes, after which I experienced chest tightness.
I felt better after drinking water and resting, and a burp seemed to relieve it.
Today, after cooking at noon and in the evening, I experienced chest tightness again.
In the afternoon, drinking water and taking deep breaths helped, but in the evening, the sensation was more pronounced and lasted longer.
I also felt a bit congested in my nose and thought I might have a cold, as I noticed a rattling sound and phlegm (white).
After dinner, I felt a sensation of something stuck in the middle of my chest (sometimes leaning to the right) between my nipples, as if the rice noodles were lodged there, causing chest tightness (similar to the feeling of choking).
When I bent down, I felt a slight urge to gag.
After about three hours, I burped a few times, and the chest tightness and difficulty breathing subsided.
Could this be related to my gastrointestinal system? What could these symptoms indicate? Additionally, a gastroenterologist I saw previously mentioned he wasn't concerned about other symptoms but was worried about my chest tightness.
Does this mean the symptoms are serious? I read online about esophageal cancer and stomach cancer, and I would like to ask for your opinion, doctor.
Thank you.

Further observation is possible, but there are concerns regarding the need for an upper gastrointestinal endoscopy.

Chest tightness can be a concerning symptom, and understanding its potential gastrointestinal causes is essential for proper diagnosis and management. Based on your description, it seems that your chest tightness may be related to gastrointestinal issues, particularly after eating certain foods or experiencing discomfort in your throat.

One possible explanation for your symptoms is gastroesophageal reflux disease (GERD). GERD occurs when stomach acid flows back into the esophagus, leading to symptoms such as chest tightness, heartburn, and a sensation of food being stuck in the throat. The fact that you experienced relief after burping and that your symptoms were triggered by eating suggests that this could be a contributing factor. Additionally, the sensation of having something stuck in your throat and the need to cough up phlegm could indicate irritation in the esophagus or throat, which can occur with reflux.

Another possibility is esophageal spasm, which can cause chest pain or tightness that may mimic heart-related issues. This condition can be triggered by stress, certain foods, or even temperature changes in food (like hot or cold items). The tightness you feel, especially after meals, could be related to the esophagus reacting to the food you consume.

Your mention of nasal congestion and phlegm could also indicate a respiratory component, such as post-nasal drip, which can lead to throat irritation and a sensation of tightness in the chest. Allergies or a mild respiratory infection could be contributing to this feeling, especially if you have been experiencing other symptoms like a cough or throat discomfort.

It’s important to note that while gastrointestinal issues are often the cause of chest tightness, other serious conditions, such as cardiac problems, should not be overlooked. If you experience severe or persistent chest tightness, especially if accompanied by other symptoms like shortness of breath, sweating, or radiating pain to the arm or jaw, it is crucial to seek immediate medical attention.

Regarding your concern about serious conditions like esophageal or stomach cancer, while it’s natural to worry, these conditions typically present with additional symptoms, such as significant weight loss, persistent vomiting, or difficulty swallowing. If your symptoms are primarily related to eating and improve with burping or changes in position, it is less likely to be cancer-related.

To manage your symptoms, consider the following recommendations:
1. Dietary Modifications: Avoid foods that trigger your symptoms, such as spicy, fatty, or acidic foods. Eating smaller, more frequent meals can also help reduce pressure on the stomach and esophagus.

2. Hydration: Ensure you are drinking enough water, but try to avoid drinking large amounts during meals, as this can increase stomach pressure.

3. Posture: After eating, try to remain upright for at least 30 minutes to help prevent reflux.

4. Stress Management: Since stress can exacerbate gastrointestinal symptoms, consider relaxation techniques such as deep breathing, meditation, or yoga.

5. Medical Consultation: If your symptoms persist or worsen, it is advisable to follow up with a gastroenterologist. They may recommend further evaluation, such as an endoscopy or esophageal motility study, to assess for conditions like GERD or esophageal spasms.

In conclusion, while your symptoms may be related to gastrointestinal issues, it is essential to monitor them closely and seek medical advice if they persist or worsen. Understanding the relationship between stress, diet, and gastrointestinal health can help you manage your symptoms effectively.
