Swelling after a car accident?

Hello Doctor, on July 2nd, I had a motorcycle accident.
Since I didn't feel it was serious, I treated myself at home.
However, in the early hours of July 6th, I noticed large bruises and unexplained swelling on my leg, so I went to the emergency room at Linkou Chang Gung Memorial Hospital.
After an X-ray and blood tests, the doctors told me there was no major issue, and I just needed to keep the wound dry to avoid infection.
But today, after getting home from work on July 6th, I noticed that my previously unbruised toes have suddenly developed bruises.
I would like to ask the doctor if I need to go back to the hospital for another check-up, or if this is just a hematoma from the impact.
I have attached pictures of my leg for your reference.
Thank you for your help!

Dr. Li Zhonghuai reply Surgery

Hello! You need to clean the wounds on your first, third, and fourth toes daily with sterile saline, then apply antibiotic ointment.
Avoid contact with tap water.
Keep your foot elevated while resting and sleeping, and apply ice.
If the swelling does not subside, you should return for a follow-up appointment.
Take care! Changhua Hospital is concerned about you!

More Info

After a motorcycle accident, it is common to experience swelling and bruising, especially in the areas that have sustained impact. In your case, you initially felt that the injury was not severe and opted for home treatment. However, the emergence of large bruises and swelling days after the incident raises some concerns that warrant further medical evaluation.

Understanding Post-Crash Symptoms
1. Bruising and Swelling: Bruising occurs when small blood vessels under the skin break due to trauma, leading to blood leaking into the surrounding tissues. Swelling can result from inflammation or fluid accumulation in response to injury. While some swelling and bruising can be expected after a fall, significant changes in the condition of your injury, such as the sudden appearance of bruising in previously unaffected areas, may indicate complications.

2. Potential Complications: The sudden development of new bruising could suggest several possibilities:
- Internal Bleeding: If the injury was more severe than initially assessed, there could be internal bleeding that is now manifesting as bruising.

- Hematoma Formation: A hematoma is a localized collection of blood outside of blood vessels, which can occur after trauma. If the hematoma grows or becomes painful, it may require medical intervention.

- Infection: Although you were advised to keep the wound dry to prevent infection, any open wounds can still become infected, leading to swelling and discoloration.

When to Seek Medical Attention
Given your symptoms, it is advisable to seek medical attention again, especially if you notice:
- Increased Swelling: If the swelling worsens or spreads.

- New Bruising: The appearance of new bruises, particularly if they are large or painful.

- Pain: If the pain increases or does not improve with rest and over-the-counter pain relief.

- Changes in Mobility: If you experience difficulty moving your foot or if there is numbness or tingling.

- Signs of Infection: Such as redness, warmth, or discharge from any wounds.

Recommended Actions
1. Return to the Hospital: Given the new symptoms, it would be prudent to return to the emergency department or consult with your primary care physician. They may perform additional imaging studies, such as an ultrasound or CT scan, to assess for any internal injuries or complications.

2. Monitor Symptoms: Keep a close watch on your symptoms. If you experience any of the warning signs mentioned above, do not hesitate to seek immediate medical care.

3. Rest and Elevation: While waiting for your next medical appointment, rest your injured foot and keep it elevated to help reduce swelling.

4. Ice Therapy: Applying ice packs to the swollen area can help manage swelling and pain. Make sure to wrap the ice in a cloth to avoid direct contact with the skin.

5. Avoid Strain: Refrain from putting weight on the injured foot until you have been evaluated by a healthcare professional.

Conclusion
In summary, while some swelling and bruising are normal after a fall, the development of new bruises and significant swelling should not be ignored. It is essential to seek further medical evaluation to rule out any serious complications. Your health and safety are paramount, and timely intervention can prevent further issues down the line. Please take care and prioritize your recovery.
