Testicular Pressure: Causes and Relief Strategies - Urology

Testicular pressure sensation

Hello Doctor: For the past few days, I have been experiencing a feeling of pressure in my testicles, along with a sensation of fullness.
However, upon examination, they do not appear to be enlarged compared to before.
I feel most uncomfortable when sitting, somewhat less so when lying down, and it is more relieved when standing.
I researched online and found that a feeling of fullness in the testicles could possibly be due to varicocele, but there are no visible symptoms that match the descriptions of varicocele.
I would like to ask the doctor what the possible issues could be and if there are any methods to alleviate the symptoms?

Niming, 20~29 year old female. Ask Date: 2022/07/11

Understanding testicular pressure and discomfort can be concerning, especially when symptoms such as a feeling of pressure or fullness arise. Based on your description, it seems that you are experiencing a sensation of pressure in the testicular area without any visible swelling or enlargement. This can be attributed to several potential causes, and it’s essential to consider these possibilities to determine the best course of action.

One common cause of testicular discomfort is varicocele, which is an enlargement of the veins within the scrotum. While you mentioned that there are no visible signs of varicocele, it can sometimes present with a feeling of heaviness or pressure, especially when sitting or standing for prolonged periods. Varicoceles are more common on the left side due to anatomical differences in venous drainage. They can lead to discomfort, and in some cases, they may affect fertility.

Another potential cause could be epididymitis, which is inflammation of the epididymis, the tube that carries sperm from the testicles. This condition can cause pain and a feeling of fullness in the scrotum. Epididymitis can be caused by infections, including sexually transmitted infections, or non-infectious causes such as trauma or certain medications.

Testicular torsion is another serious condition that can cause acute pain and pressure. However, this typically presents with severe pain and requires immediate medical attention. Since you mentioned that the discomfort is not severe and does not appear to be associated with swelling, torsion may be less likely.

Inguinal hernias can also cause a sensation of pressure in the testicular area. An inguinal hernia occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. This can lead to discomfort, especially when sitting or standing, and may sometimes be felt in the scrotum.

To relieve the discomfort you are experiencing, consider the following strategies:
1. Adjust Your Position: Since you mentioned that sitting exacerbates the discomfort, try to adjust your sitting posture or take breaks to stand and stretch. Using supportive underwear can also help alleviate pressure.

2. Cold Compress: Applying a cold compress to the scrotal area may help reduce any inflammation and provide temporary relief from discomfort.

3. Over-the-Counter Pain Relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen can help reduce pain and discomfort. However, consult with a healthcare provider before taking any medication.

4. Avoid Prolonged Sitting: If possible, reduce the amount of time spent sitting, especially in positions that may increase pressure on the groin area.

5. Hydration and Diet: Ensure you are well-hydrated and maintain a balanced diet, as dehydration and certain dietary choices can sometimes exacerbate discomfort.

6. Consult a Specialist: If the discomfort persists or worsens, it is crucial to consult a urologist. They can perform a physical examination and possibly imaging studies to determine the underlying cause of your symptoms.

In summary, while there are several potential causes for the pressure and discomfort you are experiencing in the testicular area, it is essential to monitor your symptoms closely. If you notice any changes, such as increased pain, swelling, or other concerning symptoms, seek medical attention promptly. A healthcare professional can provide a thorough evaluation and appropriate management based on your specific situation.
