Drug interactions
Hello, I have reviewed most of the questions regarding oral isotretinoin on the forum, but there are still some things I'm unclear about.
1.
I currently have oral isotretinoin 20mg, and my doctor suggested taking it every other day.
However, I have recently been experiencing insomnia and anxiety, and I have been prescribed a benzodiazepine anxiolytic like Xanax.
Is it safe to take these together in terms of liver damage? I really don't want to give up or interrupt my isotretinoin treatment, but if I continue to have insomnia, my skin becomes oily and I break out with acne...
I hope that taking the medication from the psychiatrist won't lead to unfavorable blood test results that would cause my isotretinoin to be discontinued.
2.
I feel that my underarms are a bit dark; can I use azelaic acid to lighten them? I have mostly seen discussions about glycolic acid online, but there is less information about using azelaic acid in the underarm area.
Thank you for taking the time to read this, and I appreciate your help!

When considering the combination of oral isotretinoin (commonly known as A Acid) and benzodiazepine medications such as Xanax (alprazolam), it is essential to understand the potential implications for liver health and overall treatment efficacy.
1. Drug Interactions and Liver Health: Isotretinoin is a powerful medication primarily used for severe acne. It is known to have significant effects on liver function, which is why regular liver function tests are recommended during treatment. Benzodiazepines, while not primarily metabolized by the liver in the same way as isotretinoin, can still have an impact on overall liver health, especially if taken in conjunction with other medications that stress the liver.
The concern with taking isotretinoin and benzodiazepines together is not so much a direct interaction between the two, but rather the cumulative effect on liver function. If you are experiencing anxiety and insomnia, it is crucial to manage these symptoms effectively, as stress can exacerbate acne. However, it is advisable to monitor your liver function closely through regular blood tests as recommended by your healthcare provider. If you notice any unusual symptoms such as jaundice (yellowing of the skin or eyes), dark urine, or severe fatigue, you should contact your physician immediately.
2. Managing Acne and Skin Concerns: Regarding your concern about darkening under the arms, using products like azelaic acid or alpha hydroxy acids (AHAs) can help lighten hyperpigmentation. While there is less discussion about the use of rhododendron flower acid (杜鵑花酸) specifically for this purpose, it is important to note that any topical treatment should be approached with caution, especially when you are on isotretinoin.
Isotretinoin can make your skin more sensitive, and introducing new topical agents may lead to irritation or adverse reactions. It is advisable to consult with your dermatologist before starting any new topical treatments, particularly if you are already undergoing isotretinoin therapy. They can provide tailored advice based on your skin type and the current state of your treatment.
In summary, while there is no direct contraindication for taking isotretinoin and benzodiazepines together, careful monitoring of liver function is essential. It is also crucial to manage your anxiety and insomnia effectively, as these can impact your skin condition. Always consult with your healthcare provider before making any changes to your medication regimen or introducing new topical treatments. Your health and safety should always come first, and your healthcare team is best equipped to guide you through this process.
