Can individuals experiencing dizziness or vertigo receive the COVID-19 vaccine?

Hello, doctor.
I am scheduled to receive the second dose of the AZ vaccine soon, but I have some health concerns that make me hesitant.
I would like to ask for your opinion: I have been experiencing inner ear imbalance (a slight swaying sensation when still), ear fullness, occasional dizziness with a feeling of mental fog (as if my thinking has become dull and slow), and insomnia for the past six months.
Given these conditions, is it advisable for me to receive the vaccine? Could it exacerbate or affect these symptoms?

Cai Xiansheng, 20~29 year old female. Ask Date: 2022/07/18

Dr. Hong Weibin reply Neurology

Hello: Although dizziness and body swaying are not contraindications for receiving the COVID-19 vaccine, if you have not been feeling well recently, it is advisable to prioritize your health and recover before getting vaccinated.

When considering whether to receive the COVID-19 vaccine, particularly in your case with symptoms of dizziness, ear issues, and a general feeling of unwellness, it is essential to weigh the benefits and potential risks carefully.
Firstly, the COVID-19 vaccines, including the AstraZeneca (AZ) vaccine, have been shown to be effective in preventing severe illness, hospitalization, and death from COVID-19. However, individuals with pre-existing health conditions or ongoing symptoms may have concerns about how the vaccine could affect their current state.

In your situation, the symptoms you describe—inner ear imbalance, ear fullness, occasional dizziness, and cognitive sluggishness—could be indicative of vestibular disorders or other underlying health issues. These symptoms can be distressing and may impact your daily life. It is crucial to consult with a healthcare provider, preferably an ear, nose, and throat (ENT) specialist or a neurologist, to evaluate your symptoms thoroughly before proceeding with vaccination.

The general recommendation is that if you have mild, stable symptoms that are not worsening, you may still be eligible to receive the vaccine. However, if your symptoms are severe or if you have a history of severe allergic reactions to vaccines, it is advisable to postpone vaccination until a thorough evaluation is completed.
Regarding the concern that the vaccine might exacerbate your symptoms, while some individuals do experience side effects from the vaccine—such as fever, fatigue, and headaches—these are typically short-lived and resolve within a few days. There is no strong evidence to suggest that the vaccine would specifically worsen ear-related symptoms or dizziness. However, individual reactions can vary, and it is essential to monitor your health closely after vaccination.

If you decide to proceed with the vaccination, ensure that you are well-hydrated and have had adequate rest beforehand. After receiving the vaccine, keep an eye on your symptoms and report any significant changes to your healthcare provider.
Additionally, it may be beneficial to explore other management strategies for your current symptoms. Physical therapy, particularly vestibular rehabilitation therapy, can be effective for managing balance issues and dizziness. Cognitive behavioral therapy (CBT) may help with feelings of anxiety or cognitive sluggishness.
In summary, while it is generally safe for individuals with mild, stable symptoms to receive the COVID-19 vaccine, it is crucial to consult with a healthcare professional who can assess your specific situation. They can provide personalized advice based on your health history and current symptoms. Ultimately, the decision should be made in collaboration with your healthcare provider, ensuring that you feel comfortable and informed about your choice.
