Bowel movements and gastrointestinal-related issues?

Hello Doctor: 1) Occasionally, my stools are harder, accompanied by pain, slight bleeding, and anal itching.
Could this be a symptom of anal fissure? 2) Sometimes the color of my stools is darker (the shape is intact but appears black), while the color on the toilet paper looks fine; when I drink yogurt, the color is lighter or normal.
Is this symptom abnormal? 3) I have a lean physique (height 181 cm / weight 63 kg), and several years ago, I noticed that when lying flat and applying pressure to my lower left abdomen, I could feel a structure extending from the upper left towards the lower middle, resembling an intestinal loop, with a slight irregularity.
It becomes less noticeable after a bowel movement.
Is this condition normal? I would greatly appreciate your guidance.

Mr. Hsu, 20~29 year old female. Ask Date: 2022/07/19

When it comes to bowel issues, particularly symptoms like anal fissures, it's essential to understand the underlying causes and implications of the symptoms you described. Let's break down your concerns systematically.

1. Symptoms of Anal Fissures: The symptoms you mentioned—hard stools, pain during bowel movements, slight bleeding, and itching around the anus—are indeed indicative of anal fissures. An anal fissure is a small tear in the lining of the anus, often caused by passing hard or large stools. The pain can be quite severe, especially during and after bowel movements, and the presence of blood can be alarming. The itching may occur due to irritation from the fissure or from the use of toilet paper. It's crucial to manage this condition by ensuring softer stools through dietary changes, increased hydration, and possibly the use of stool softeners. If the fissure persists, further medical intervention may be necessary.

2. Bowel Movement Color: The observation of darker stools can be concerning, especially if they appear black. Black stools can indicate the presence of blood that has been digested, which could suggest a bleeding source higher up in the gastrointestinal tract. However, certain foods, medications (like iron supplements), or even certain health conditions can also cause darker stools. The fact that your stools appear normal after consuming yogurt suggests that your diet may be influencing your stool color. It's essential to monitor this symptom closely. If you notice persistent changes in stool color or consistency, or if you experience additional symptoms like abdominal pain or changes in appetite, you should consult a healthcare provider for further evaluation.

3. Palpable Abdominal Mass: The sensation of a palpable structure in your left lower abdomen could be related to several factors. It might be a normal anatomical structure, such as a loop of bowel, especially if it becomes less noticeable after a bowel movement. However, if this mass is new, persistent, or associated with pain, changes in bowel habits, or other gastrointestinal symptoms, it warrants further investigation. Conditions such as hernias, tumors, or other gastrointestinal issues could present as palpable masses. Given your concern, it would be prudent to discuss this finding with your healthcare provider, who may recommend imaging studies like an ultrasound or CT scan to assess the area more thoroughly.

In summary, your symptoms could indeed be related to anal fissures, especially given the pain and bleeding during bowel movements. The color of your stools should be monitored, and any persistent changes should be evaluated by a healthcare professional. Lastly, the palpable mass in your abdomen should be assessed to rule out any significant underlying conditions.
To support your bowel health, consider the following recommendations:
- Dietary Adjustments: Increase fiber intake through fruits, vegetables, and whole grains to help soften stools. Adequate hydration is also crucial.

- Regular Exercise: Physical activity can promote regular bowel movements and overall digestive health.

- Stress Management: Since anxiety can exacerbate gastrointestinal symptoms, incorporating stress-reducing techniques such as mindfulness, yoga, or therapy may be beneficial.

If symptoms persist or worsen, please seek medical advice promptly to ensure appropriate management and care.
