Post-anal fistula surgery

Dear Dr.
Chang,
I hope this message finds you well.
It has been a month since my fistula excision surgery, and during my follow-up visit the day before yesterday, the doctor mentioned that my recovery is progressing well and that I no longer need to return for follow-up appointments.
I also no longer need to use Biomycin ointment.
However, I have two questions that I forgot to ask:
1.
There is still a slight amount of tissue fluid oozing from the wound.
Can I start using body wash to clean it, or should I continue with warm water sit baths and gauze dressing? If so, how much longer should I maintain this?
2.
Given the current situation, am I cleared to resume activities such as jogging or swimming, or should I wait a bit longer?
Thank you, and I apologize for not asking these questions during my appointment.

Nathan, 20~29 year old female. Ask Date: 2022/07/23

1.
If there is still exudate from the wound, it indicates that it has not fully healed.
You can clean it normally, pat it dry, and apply a small amount of ointment.
Maintain this until the wound is completely dry.

2.
You can engage in activities, but you should avoid prolonged friction on the wound.
Start with short distances and gradually return to your original training volume if there is no discomfort.

Post-surgery care for an anal fistula is crucial for ensuring proper healing and preventing complications. After a month of recovery, it’s great to hear that your doctor has noted positive progress. Here are some detailed guidelines regarding wound care and exercise recommendations.

Wound Care
1. Cleaning the Wound:
- It is essential to keep the surgical site clean to prevent infection. If there is still a slight discharge of tissue fluid, it is advisable to continue with gentle cleaning. You can use warm water for sitz baths, which can help soothe the area and promote healing.
- If your doctor has not specifically advised against it, you may start using a mild, unscented body wash or shower gel, but be cautious. Avoid scrubbing the area directly. Instead, let the water run over the area and gently pat it dry with a clean towel.

- Continue to use gauze or a clean cloth to cover the area if there is any discharge. Change the dressing as needed to keep the area dry and clean.

2. Duration of Care:
- The duration for which you should maintain sitz baths and dressing will depend on the amount of discharge and your comfort level. If the discharge decreases significantly, you may be able to transition to regular bathing. However, if you notice any increase in discharge, redness, or pain, it’s best to revert to sitz baths and consult your doctor.

Exercise Guidelines
1. Resuming Activities:
- After anal fistula surgery, it is generally recommended to avoid strenuous activities for at least 4-6 weeks post-operation. This includes high-impact exercises like running and swimming, which can put pressure on the surgical site and potentially disrupt healing.

- Light activities such as walking can usually be resumed sooner, as they promote circulation and healing without excessive strain.

2. Slowly Introducing Exercise:
- If you feel comfortable and have no significant pain or discomfort, you might start with gentle stretching or low-impact exercises. Gradually increase the intensity as you feel more confident in your recovery.

- For running or swimming, it’s best to wait until you have fully healed and received clearance from your healthcare provider. This might take a few more weeks, depending on your individual healing process.

3. Listening to Your Body:
- Pay attention to how your body responds to any activity. If you experience pain, discomfort, or any unusual symptoms, stop the activity and consult your doctor.

Additional Considerations
- Diet and Hydration: Ensure you maintain a high-fiber diet and stay hydrated to prevent constipation, which can strain the surgical site during bowel movements.

- Follow-Up Care: Although your doctor has indicated that no further follow-up is necessary, if you have any concerns or if the wound does not appear to be healing properly, do not hesitate to reach out for further evaluation.

In conclusion, while it’s encouraging that you are healing well, it’s important to proceed with caution regarding both wound care and the resumption of physical activities. Always prioritize your comfort and consult your healthcare provider for personalized advice tailored to your recovery process.
