Penile Shape Changes During Urination: Is It Normal? - Urology

Penile issues

The doctor previously posted an article mentioning that when waking up at night to urinate, the penis is in a semi-erect state, and it feels thicker near the glans (the head of the penis) while thinner closer to the pubic area.
I would like to clarify that the shape I described is not referring to the entire penis being in a "U" shape, but rather that it resembles two inverted "U" shapes, with the upper part being thicker and the lower part being thinner.
There seems to be a noticeable difference in thickness in the middle, but it is not very pronounced.
However, if I slightly push the foreskin towards the glans, this difference disappears, and it does not occur when fully erect.
I would like to ask again if this is normal.
Could it be due to having a long foreskin, or is it because the foreskin at the tip is too thin?

Hello: It is quite common for the foreskin to cause visual effects.
Wishing you good health.

Understanding changes in penile shape during urination, particularly when the penis is in a semi-erect state, can be a source of concern for many men. The observations you've made regarding the shape of your penis—where the area near the glans (the head) appears thicker compared to the base—are not uncommon and can be attributed to several physiological factors.

Firstly, it is essential to recognize that the penis is composed of erectile tissue, which can expand and contract based on various factors, including blood flow, arousal, and even the position of the body. When you wake up to urinate, the penis may not be fully erect, leading to a semi-erect state. In this state, the distribution of blood within the erectile tissues can create a visual effect where the glans appears fuller or thicker compared to the shaft. This is a normal physiological response and does not indicate any underlying health issues.

The shape you described, resembling an inverted "U," where the upper part is thicker and the lower part is thinner, can also be influenced by the presence of the foreskin. If the foreskin is longer or tighter, it may create a visual impression of unevenness when the penis is in a semi-erect state. When you push the foreskin back, it may allow the penis to appear more uniform, which suggests that the foreskin's position can indeed affect the perceived shape of the penis.

Regarding your concerns about whether this is due to a long foreskin or a thin foreskin at the tip, both factors can play a role. A longer foreskin may not retract fully during a semi-erect state, leading to a more pronounced difference in thickness. Conversely, if the foreskin is thin or tight, it may not allow for the same degree of expansion, which can also contribute to the visual effect you are observing.

It's important to note that as long as you are not experiencing pain, discomfort, or any functional issues during erections or sexual activity, these variations in shape are generally considered normal. The penis can have different shapes and sizes, and slight variations in appearance are common among men.
If you are still concerned about the appearance of your penis or if you notice any changes that cause discomfort or pain, it would be advisable to consult with a healthcare professional, such as a urologist. They can provide a thorough examination and address any specific concerns you may have regarding your penile health.

In summary, the changes in penile shape you are observing during urination in a semi-erect state are likely normal and can be influenced by factors such as blood flow, the position of the foreskin, and the natural anatomy of the penis. As long as there are no accompanying symptoms of pain or dysfunction, there is typically no cause for concern. However, if you have persistent worries, seeking medical advice is always a prudent choice.
