Penile issues
Hello Doctor: I would like to ask about the following situation.
When I wake up in the morning and am in a slightly erect state, the shaft appears to have a thicker part near the glans and the lower half of the shaft, while the area closer to the base is thinner.
There seems to be a noticeable transition in thickness, and it feels different to the touch, but there is no pain or discomfort, and it does not affect my erection.
However, when fully erect, the thickness becomes uniform again, and after waking up, there is no variation in thickness.
This discrepancy only occurs when I wake up from sleep.
If I push the foreskin slightly towards the glans during this situation, it returns to its normal appearance.
Is there any issue with this? Could it be due to a long foreskin? If I don't get it checked, could it lead to any complications? I have researched paraphimosis, which seems to involve the foreskin being stuck behind the glans and unable to be retracted, but I do not experience that situation, so it should not be paraphimosis, right? (The foreskin is never stuck or unable to move.)

Your description seems fine, but I don't quite understand the part about the foreskin.
If you have concerns, I recommend visiting a urologist for a thorough examination.

Morning erections, also known as nocturnal penile tumescence, are a normal physiological phenomenon that occurs during sleep, particularly during the REM (Rapid Eye Movement) phase. It is common for men to experience variations in the appearance and firmness of their erections upon waking. The situation you described, where the penis appears thicker at the head and thinner towards the base, is not unusual and can be attributed to several factors.
Firstly, the penis is composed of erectile tissue that fills with blood during an erection. The distribution of blood flow can vary based on several factors, including the position of the body during sleep, hormonal fluctuations, and even the state of relaxation of the surrounding muscles. When you wake up, the blood flow may not be evenly distributed, leading to the appearance of a "thicker" head and a "thinner" base. This is generally not a cause for concern, especially if you do not experience pain or discomfort.
Regarding the concern about phimosis or tight foreskin, it is important to differentiate between normal anatomical variations and pathological conditions. Phimosis occurs when the foreskin cannot be retracted over the glans (head) of the penis. In your case, since you mentioned that you can push the foreskin back without any difficulty and that there is no pain or discomfort, it is unlikely that you are experiencing phimosis. The fact that the penis returns to a normal appearance upon full erection further supports this.
It is also worth noting that variations in the appearance of the penis can occur due to factors such as temperature, arousal levels, and even anxiety. If you are not experiencing any pain, discomfort, or functional issues (such as difficulty achieving or maintaining an erection), it is generally safe to conclude that what you are experiencing is a normal variation.
However, if you notice any changes in your sexual function, persistent pain, or if the appearance of your penis changes significantly over time, it would be advisable to consult a healthcare professional. Regular check-ups with a urologist can help address any concerns and ensure that everything is functioning normally.
In summary, the variations you are experiencing in the appearance of your penis during morning erections are likely normal and not a cause for concern. As long as you are not experiencing pain or functional issues, there is no immediate need for alarm. However, maintaining open communication with a healthcare provider about any changes or concerns is always a good practice for your overall health and well-being.
