Penile issues
Hello doctor, I would like to ask about a small area on my penis, located between the base and the glans, which experiences pain similar to that of a bruise when pressed.
It does not hurt during normal times or when fully erect.
There are no visible issues on the surface, but if I wear tighter pants and my penis shifts to one side, I feel pain.
Since I have not undergone circumcision, I also experience pain during sexual activity when I attempt to manipulate it before achieving full erection, possibly due to the pulling of the foreskin.
However, once fully erect, as long as I do not press on that area, there are no significant problems.
What could be causing this?

It sounds like chronic microtrauma leading to fibrosis caused by chronic inflammation.
It is advisable to consult a urologist for a thorough examination by a physician.

More Info
It sounds like you are experiencing localized pain in the penis, particularly in the area between the base and the glans (tip). This type of pain can be concerning, and while it may not be indicative of a serious issue, it is essential to understand the potential causes and solutions.
Possible Causes of Pain in the Penis
1. Trauma or Injury: If you have experienced any trauma to the area, even minor, it could lead to localized pain. This could be from vigorous sexual activity, rough handling, or even tight clothing that may have caused some irritation or micro-injuries.
2. Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, which can cause curved, painful erections. Although you mentioned that the pain does not occur during a full erection, it is still worth considering if there is any curvature or abnormality in the shape of your penis.
3. Infection or Inflammation: Conditions such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin) can cause discomfort. Even if there are no visible signs of infection, underlying inflammation could lead to pain, especially during movement or pressure.
4. Nerve Irritation or Compression: Sometimes, nerve issues can cause localized pain. This could be due to pressure from tight clothing or a position that irritates the nerves in that area.
5. Skin Conditions: Although you mentioned that there are no visible issues, it’s still possible that there could be a skin condition affecting the area, such as dermatitis or a fungal infection, which might not be immediately apparent.
Recommendations for Management
1. Avoid Tight Clothing: Since you mentioned that wearing tighter pants exacerbates the pain, consider wearing looser-fitting clothing to reduce pressure on the area.
2. Gentle Handling: Be cautious during sexual activity and avoid any positions or movements that cause discomfort. If you suspect that the pain is related to the pulling of the foreskin, consider discussing the possibility of a circumcision with a urologist, as this can sometimes alleviate issues related to tight foreskin.
3. Warm Compresses: Applying a warm compress to the area may help alleviate discomfort and promote blood flow, which can aid in healing if there is any minor injury or inflammation.
4. Over-the-Counter Pain Relief: Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help reduce pain and inflammation. However, consult with a healthcare provider before taking any medication.
5. Consult a Urologist: Given that the pain occurs during specific activities and is localized, it would be wise to consult a urologist. They can perform a physical examination and may recommend imaging studies if necessary to rule out any underlying conditions.
6. Monitor Symptoms: Keep track of when the pain occurs, its intensity, and any other symptoms that may arise. This information will be helpful for your healthcare provider in diagnosing the issue.
Conclusion
While localized pain in the penis can be distressing, it is often manageable with appropriate care and lifestyle adjustments. However, it is crucial to seek professional medical advice to rule out any serious conditions and to receive tailored treatment. Remember, open communication with your healthcare provider about your symptoms is key to finding the right solution.
