Irregular menstrual cycle?

Hello, doctor.
My menstrual period started on the 10th of August.
I had several sexual encounters from the 17th to the 20th, and afterward, I experienced a continuous light discharge that was brownish and black, resembling spotting.
After the 21st, I did not have any sexual activity.
Starting on the 23rd, I noticed bright red bleeding, and the amount has significantly increased.
As of today, the 25th, I am still experiencing bleeding.
What should I do?

Lin, 20~29 year old female. Ask Date: 2022/08/25

Dr. Cao Guotao reply Obstetrics and Gynecology

Hello, there seems to be something unusual.
I recommend seeing a gynecologist.
Wishing you good health.

Understanding irregular menstrual cycles can be quite complex, as they can be influenced by a variety of factors including hormonal changes, stress, lifestyle, and underlying health conditions. Based on your description, it seems that you experienced a series of changes in your menstrual cycle following sexual activity, which has led to some confusion regarding the nature of your bleeding.

Firstly, it is important to note that menstrual cycles can vary significantly from person to person and even from month to month for the same individual. A typical menstrual cycle lasts between 21 to 35 days, and bleeding can last anywhere from 2 to 7 days. However, irregularities can occur due to several reasons.

1. Hormonal Fluctuations: The menstrual cycle is regulated by hormones such as estrogen and progesterone. Any disruption in the hormonal balance can lead to irregular bleeding. Factors such as stress, significant weight changes, excessive exercise, or hormonal contraceptives can all contribute to these fluctuations.

2. Implantation Bleeding: If you had unprotected sexual intercourse, there is a possibility of pregnancy. Implantation bleeding can occur when a fertilized egg attaches to the lining of the uterus, usually around 6 to 12 days after conception. This type of bleeding is typically light and can be brown or pink in color.

3. Menstrual Irregularities: Conditions such as polycystic ovary syndrome (PCOS), fibroids, or endometriosis can cause irregular menstrual cycles and abnormal bleeding patterns. If you have a history of irregular periods or other symptoms like pelvic pain, it may be worth discussing these conditions with your healthcare provider.

4. Infections or Other Medical Conditions: Sometimes, infections or other medical conditions can lead to abnormal bleeding. Conditions such as pelvic inflammatory disease (PID) or sexually transmitted infections (STIs) can cause changes in your menstrual cycle and should be evaluated by a healthcare professional.

Given that you have experienced a change in your menstrual bleeding pattern, it is advisable to monitor your symptoms closely. If the bleeding continues, becomes heavier, or is accompanied by severe pain, it is important to seek medical attention. A healthcare provider may recommend a pelvic exam, ultrasound, or blood tests to determine the underlying cause of your symptoms.

In the meantime, here are some steps you can take:
- Track Your Cycle: Keep a record of your menstrual cycle, including the start and end dates, the flow (light, moderate, heavy), and any other symptoms you experience. This information can be helpful for your healthcare provider.

- Manage Stress: Since stress can impact your menstrual cycle, consider incorporating stress-reducing activities into your routine, such as yoga, meditation, or regular exercise.

- Healthy Lifestyle: Maintain a balanced diet and regular exercise routine, as these can help regulate your menstrual cycle.

- Consult a Healthcare Provider: If your symptoms persist or worsen, do not hesitate to reach out to a healthcare professional for further evaluation and guidance.

In conclusion, while irregular menstrual cycles can be concerning, they are often manageable with the right approach and medical guidance. It is essential to listen to your body and seek help when needed.
