Is it menopause?
Hello Doctor, my mother is 44 years old and her menstrual cycle has been irregular for the past few months, sometimes arriving early and other times late.
The duration of her periods also varies, sometimes lasting up to two weeks.
Could this be due to menopause, or could there be other underlying conditions? I would appreciate your insights.

Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the factors affecting the menstrual cycle are numerous, including age, environment, lifestyle, stress, emotional instability, weight changes, medications, emergency contraceptives, diet, polycystic ovary syndrome, lesions in the uterus or cervix, infections, ovulation, pregnancy, miscarriage, and breastfeeding, among others.
It is advisable to consult a gynecologist if symptoms persist or worsen to determine if there is a hormonal imbalance or other underlying causes.
Thank you! Wishing you peace and health, Dr.
Chang.

Irregular menstruation can indeed be a sign of approaching menopause, especially in women around the age of 44. This transitional phase, known as perimenopause, typically begins several years before menopause, which is defined as the cessation of menstruation for 12 consecutive months. During perimenopause, hormonal fluctuations can lead to irregular menstrual cycles, changes in flow, and varying cycle lengths.
In your mother's case, the irregularities she is experiencing—such as periods arriving earlier or later than expected, and varying lengths of menstruation—are common symptoms of perimenopause. The average age for menopause in women is around 51, but the transition can start in the 40s for some women. During this time, estrogen and progesterone levels fluctuate, which can affect the menstrual cycle.
However, while irregular menstruation can be a normal part of the perimenopausal transition, it is essential to consider other potential causes for these changes. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, uterine fibroids, or even stress and significant lifestyle changes can also lead to irregular periods. Therefore, it is crucial for your mother to consult with a healthcare provider to rule out any underlying medical conditions.
In addition to menstrual irregularities, women in perimenopause may experience other symptoms, including hot flashes, night sweats, mood swings, sleep disturbances, and vaginal dryness. These symptoms are also related to hormonal changes and can vary in intensity and duration.
If your mother is concerned about her symptoms, it would be beneficial for her to schedule an appointment with her healthcare provider. The provider may perform a physical examination, review her medical history, and possibly conduct blood tests to check hormone levels. This comprehensive approach can help determine whether her symptoms are indeed related to perimenopause or if there are other factors at play.
In summary, while irregular menstruation at the age of 44 can be a sign of approaching menopause, it is essential to consider other possible causes. A thorough evaluation by a healthcare professional can provide clarity and guide appropriate management options. If her symptoms are related to perimenopause, there are various treatment options available, including lifestyle modifications, hormonal therapies, and non-hormonal medications to alleviate symptoms.
