Abnormal menstrual cycle after abortion?

Hello doctor, I would like to inquire about my situation.
I became pregnant on July 7 in Japan, but due to fetal growth cessation, I underwent a dilation and curettage (D&C) procedure.
My menstrual period started on August 4, which was quite heavy but lasted about a week, and it seemed normal.
My second menstrual period was expected to start on September 4, but I began to notice a small amount of brown discharge on September 1, which was only enough to fill a panty liner.
I was quite anxious and went to see a gynecologist.
The doctor performed an ultrasound on my uterus and said that everything was healing well.
He advised me to start trying to conceive again after my October menstrual period.
The doctor prescribed me 14 days of progesterone (Utrogestan) to take one tablet daily.
However, my menstrual flow has been slightly heavier than before and gradually turned red, although there is still a small amount of brown discharge.
Now, my period has lasted almost two weeks without stopping, and I am unsure what to do.
I am very worried about how this might affect my future attempts to conceive...

Hello, sometimes menstruation can be irregular after a miscarriage, but if you have been bleeding for two weeks, that is quite a long time.
It is advisable to return to the obstetrics and gynecology department for a check-up.
Wishing you good health.

After undergoing an abortion, it is common for women to experience menstrual irregularities, which can include changes in flow, timing, and color of menstrual blood. Your situation, where you had a normal first period after the procedure but are now experiencing prolonged bleeding and changes in the color of your menstrual blood, is understandably concerning. Here’s a detailed overview of what to expect post-abortion and when to seek further medical attention.

What to Expect After an Abortion
1. Initial Recovery: After an abortion, it is typical for the first menstrual period to occur within 4 to 6 weeks. This period may be heavier than usual, as the body sheds the uterine lining that had built up during pregnancy. It sounds like your first period after the abortion was normal, lasting about a week with a heavy flow.

2. Subsequent Cycles: The second menstrual cycle can be unpredictable. It may come earlier or later than expected, and the flow can vary. The presence of brown blood often indicates older blood that has taken longer to exit the uterus, which is usually not a cause for concern. However, if you notice a significant change in your cycle, such as prolonged bleeding or an increase in pain, it is essential to consult your healthcare provider.

3. Hormonal Changes: The medication you were prescribed, likely a form of progesterone (like the one you mentioned, "益斯德錠"), is intended to help regulate your menstrual cycle and prepare your body for future pregnancies. Hormonal treatments can sometimes cause changes in bleeding patterns, including heavier or prolonged periods.

When to Worry
1. Prolonged Bleeding: If your menstrual bleeding lasts longer than two weeks or if you are soaking through pads or tampons every hour for several consecutive hours, this could indicate a complication such as retained products of conception or an infection. In such cases, it is crucial to seek medical attention promptly.

2. Severe Pain: While some cramping is normal, severe pain that does not improve with over-the-counter pain relief should be evaluated by a healthcare provider.

3. Signs of Infection: Symptoms such as fever, chills, or foul-smelling discharge can indicate an infection, which requires immediate medical evaluation.

4. Emotional Well-being: It’s also important to consider your emotional health after an abortion. Feelings of anxiety or depression are common, and if you find these feelings overwhelming, it may be beneficial to speak with a mental health professional.

Next Steps
Given that your bleeding has persisted for almost two weeks and has changed in character, it is advisable to follow up with your healthcare provider. They may recommend an ultrasound to ensure that there are no retained products in the uterus and to assess the overall health of your reproductive system.
Additionally, if you are considering trying to conceive again soon, it is essential to ensure that your body is fully healed and that your menstrual cycle has returned to a regular pattern. Your doctor can provide guidance on when it is safe to start trying for another pregnancy based on your individual recovery.

Conclusion
In summary, while some irregularities in your menstrual cycle after an abortion are normal, prolonged bleeding and changes in flow warrant further investigation. It is crucial to maintain open communication with your healthcare provider and to seek help if you have any concerns about your recovery or future fertility. Your health and well-being are paramount, and addressing any issues early can help ensure a smoother path to future pregnancies.
