Prolonged menstruation

Hello, I would like to ask about my menstrual cycle.
My period lasts for about 10 days, with the first 5 to 6 days showing a small amount of a color similar to brown (so little that I only need to use panty liners).
However, around days 7 to 8, the flow increases and becomes bright red until the end.
Overall, my menstrual period lasts about 10 days, and this has been bothering me.
What could this symptom indicate?

kaka, 40~49 year old female. Ask Date: 2022/09/12

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the factors that may affect the duration of a woman's menstrual period include hormonal imbalances, emotions, diet, age, and others.
However, it is most important to rule out the presence of uterine tumors, endometrial hyperplasia, cervical lesions, vaginal lesions, and so on.
If the symptoms persist, it is advisable to consult a physician for further evaluation.
Thank you! Wishing you peace and health.

Prolonged menstrual periods, also known as menorrhagia, can be a source of concern for many individuals. Your description of experiencing menstrual bleeding that lasts for over ten days, with the first few days characterized by light brown discharge followed by heavier bright red bleeding, suggests that you may be experiencing a variation of normal menstruation, but it could also indicate an underlying issue that warrants further investigation.

There are several potential causes for prolonged menstrual bleeding. Hormonal imbalances are one of the most common reasons. The menstrual cycle is regulated by a complex interplay of hormones, including estrogen and progesterone. If these hormones are out of balance, it can lead to irregularities in the menstrual cycle, including prolonged bleeding. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or perimenopause can contribute to these hormonal fluctuations.

Another possible cause is the presence of uterine fibroids or polyps. These are benign growths in the uterus that can lead to heavier and prolonged menstrual bleeding. Endometriosis, a condition where the tissue similar to the lining of the uterus grows outside of it, can also cause prolonged periods and significant discomfort.

Infections of the reproductive system, such as pelvic inflammatory disease (PID), can lead to abnormal bleeding patterns as well. Additionally, certain medications, particularly anticoagulants or hormonal contraceptives, can affect menstrual flow and duration.

Given your symptoms, it is essential to consult with a healthcare provider for a thorough evaluation. They may recommend a pelvic examination, ultrasound, or blood tests to assess hormone levels and rule out any structural abnormalities in the uterus. It is also important to discuss your complete medical history, including any other symptoms you may be experiencing, as this can provide valuable context for your healthcare provider.

In the meantime, keeping a menstrual diary can be helpful. Documenting the length of your periods, the flow (light, moderate, heavy), and any associated symptoms (such as pain, mood changes, or fatigue) can provide your healthcare provider with useful information to make an accurate diagnosis.

Treatment options for prolonged menstrual periods depend on the underlying cause. If hormonal imbalances are identified, hormonal therapies such as birth control pills or hormone replacement therapy may be recommended. If fibroids or polyps are present, surgical options may be considered. Lifestyle changes, including diet and exercise, can also play a role in managing symptoms.

In conclusion, while prolonged menstrual periods can be a common experience for many, it is crucial to seek medical advice to rule out any serious conditions and to receive appropriate treatment. Your health and well-being are paramount, and addressing these symptoms early can lead to better outcomes and improved quality of life.
